摘　　要：目的探讨母乳成分在纯母乳喂养婴幼儿生长发育速率中的应用价值。方法选择2020年1-12月我院治疗的纯母乳喂养足月婴幼儿56例,根据婴儿生长发育速率的不同分为正常发育组(28例)和非正常发育组(28例)。统计两组婴幼儿的基本信息,并分析母乳成分,采用Logistic多元回归分析影响婴幼儿生长发育速率的因素。结果两组婴儿出生时体重、身高及喂养时间间隔、婴幼儿吮吸时间及母乳成分中乳糖含量相近,差异无统计学意义(均P>0.05);正常发育组3月龄时体重、身高、体重净增量、身高净增量及母乳成分中蛋白质、脂肪、总能量含量均高于非正常发育组,母乳中干物质含量低于非正常发育组,差异均有统计学意义(均P<0.05)。Logistic回归分析显示,母乳成分中蛋白质和脂肪含量与婴幼儿体重净增量相关,且蛋白质含量与婴幼儿身高净增量相关(P<0.05)。结论纯母乳喂养婴幼儿生长发育速率与母乳成分中蛋白质、脂肪等主要营养物质含量相关,应重视母乳各营养成分的含量,提高母乳质量,促进婴幼儿正常生长。Objective To investigate the application value of breast milk components in the growth and development rate of exclusively breastfed infants.Methods From January to December 2020,56 cases of full-term infants who were exclusively breast-fed in our hospital were selected and divided into normal development group(28 cases)and abnormal development group(28 cases)according to the different growth rate of infants.The basic information of the two groups of infants were analyzed,and the milk composition was analyzed.The factors affecting the growth rate of infants were analyzed by Logistic multiple regression.Results There were no significant differences in birth weight,height,feeding interval,sucking time and lactose content in breast milk between the two groups(all P>0.05).Body weight,height,net increment of body weight,net increment of height,protein,fat and total energy contents in breast milk of normal subjects were higher than those of abnormal subjects at 3 months,and dry matter content in breast milk of normal subjects was lower than that of abnormal subjects,the differences were statistically significant(all P<0.05).Logistic regression analysis showed that protein and fat contents in breast milk were correlated with net increment of body weight,and protein content was correlated with net increment of body height(P<0.05).Conclusion The growth and development rate of exclusively breastfed infants is related to the content of protein,fat and other main nutrients in breast milk.It is necessary to pay attention to the content of nutrients in breast milk to improve the quality of breast milk and promote the normal growth of infants.
