摘  要:Ilizarov骨搬移技术是20世纪骨外科史上的里程碑式技术之一。Ilizarov教授很早就认识到骨牵引不仅具有良好的成骨作用,而且具有优秀的成血管作用。他曾开展狗胫骨的横向骨搬移实验,证实这一技术能促进骨块周围形成新生血管,并将其用于治疗下肢慢性缺血性疾病。目前,胫骨横向骨搬移技术的临床应用还比较局限,主要用于治疗终末期的下肢血栓闭塞性脉管炎、下肢动脉硬化性闭塞症和糖尿病足,取得了肯定的疗效。手术创伤较大及其血管新生机制不明限制了它在其它缺血性疾病中的应用。本文将系统性回顾胫骨横向骨搬移技术的起源、手术方法、应用范围和作用机制,并探讨其未来的应用和研究方向。The Ilizarov technique is considered to be one of the milestones of orthopedic surgery in the 20th century.Dr.Ilizarov realized the vasculogenic effect of bone distraction besides the osteogenic effect very early.He even applied the transverse tibial bone transport(TTT)technique to clinical use to widen thin bones or to treat patients with disorders resulting from chronic ischemia,but unfortunately the detailed records of those outstanding work are no longer available.In fact,the TTT technique has been proved to be very effective in the treatment of some ischemic diseases,such as thromboangiitis obliterans,arteriosclerosis obliterans and diabetic foot ulcers.However,the complexity of TTT procedure limits its promotion in more hospitals.The lack of knowledge of its vasculogenic mechanism limits its application in other ischemic diseases.This review would give an overview of the origin,the indications and the mechanism of the TTT technique,and to seek out what to do in its further clinical application and research.
