腹腔镜十二指肠局部切除术安全性及有效性初探

摘　　要：目的探讨腹腔镜下十二指肠局部切除术的安全性和有效性。方法回顾性分析内蒙古医科大学附属医院肝胆外科2例经腹腔镜十二指肠局部切除术治疗的患者完整资料,总结患者术后疗效及并发症等相关情况,分析腹腔镜十二指肠局部切除术的可实施性。结果2例患者均在腹腔镜下成功完成十二指肠局部切除术,手术时间100-120 min,术中出血量50-100 mL,经手术治疗后,2例患者均无围术期死亡,未发生吻合口瘘、出血、感染、肠梗阻等并发症,且均于术后7 d出院。随访半年,身体状况良好,体重增加。结论腹腔镜下十二指肠局部切除术可以成为非乳头区良性病变或潜在恶性病变的重要治疗手段之一,经过精心准备,该术式具有一定的安全性及可操作性。Objective To investigate the safety and effectiveness of laparoscopic local duodenal resection.Methods A retrospective analysis was conducted in the complete data of 2 patients underwent laparoscopic local duodenal resection in the Affiliated Hospital of Inner Mongolia Medical University.By targeted surgery and perioperative care,the postoperative curative effect and complications of 2 patients were summarized,and the feasibility of laparoscopic local duodenal resection was analyzed.Results Both 2 patients successfully completed local duodenal resection under laparoscopic surgery.The operation time was 100-120 min,and the intraoperative blood loss was 50-100 mL.After surgical treatment,there was no perioperative death in 2 patients.No complication such as anastomotic fistula,bleeding,infection,or intestinal obstruction occurred,and 2 patients were discharged 7 d after surgery.After half a year of follow-up,patients were in good physical condition and gained weight.Conclusion Laparoscopic local resection of the duodenum can be one of the important treatments for benign or potentially malignant lesions in the non-papillary area.After careful preparation,this procedure has a certain degree of safety andoperability.
