鼻内镜术后不同鼻腔填塞方式的疗效评价

摘　　要：目的探究鼻内镜术后应用纳西绵、膨胀海绵及凡士林纱条填塞的疗效评价。方法选择2019年1—12月本院收治的110例实施鼻内镜手术患者作为研究对象,随机数表法分为纳吸绵组、膨胀海绵组、纳西绵+膨胀海绵组、纳西绵+凡士林组和膨胀海绵+凡士林组共五组,每组各22例。比较不同填塞方式鼻腔填塞期、鼻腔填塞物取出过程出血量及引起不适症状VAS评分。结果纳西绵+凡士林组和膨胀海绵+凡士林组在鼻腔填塞期出血量最少,与其余三组比较差异有统计学意义(P<0.05);纳西绵组在鼻腔填塞期出血量最多,在鼻腔填塞物取出过程出血量最少,填塞引起的鼻(头)痛、睡眠质量、填塞物取出过程疼痛VAS评分最低,与其余四组比较差异有统计学意义(P<0.05)。结论纳西绵是一种良好的鼻内镜术后鼻腔填塞材料,具有可吸收、减轻不适感、止血效果好等优点,具有明显的临床疗效。对于术后出血风险较高的鼻内镜手术,可考虑行联合填塞,以降低术后出血风险。Objective To evaluate the efficacy of nasopore,expansion sponge and vaseline gauze in nasal filling after nasal endoscopy.Methods Select 110 cases ofhospitalized patients those received nasal endoscopic surgery in our hospital between January 2019 and December 2019 as research subjects,and they were randomly divided into nasopore group,expansion sponge group,nasopore plus expansion sponge group,nasopore plus vaseline gauze group,and expansion sponge plus vaseline gauzegroup,22 cases in each group.The nasal cavity filling period,and blood loss and VAS scores of symptoms during the extract of nasal cavity filling stuffs were compared.Results Nasopore plus vaseline gauzegroup and expansion sponge plus vaseline gauzegroup had the least amount of blood loss in the nasal cavity filling stage,and the difference was statistically significant when compared with the other three groups(P<0.05).The nasopore group had the largest amount of blood loss in the nasal cavity filling stage,and the smallest amount of blood loss in the nasal cavity extract process.The nasal(head)pain caused by the filling,the sleep quality and the VAS score during the extract process were the lowest,and the differences were statistically significant when compared with the other four groups(P<0.05).Conclusions Nasopore is a good nasal filling material after nasal endoscopic surgery,with the advantages of absorption,relief of discomfort,good hemostasis,it has obvious clinical efficacy.For nasal endoscopic surgery with high risk of postoperative bleeding,combined tamponade may be considered to reduce the risk of postoperative bleeding.
