摘　　要：目的本研究旨在研究多系统萎缩(MSA)伴轻度认知功能障碍(MCI)患者皮层的形态学改变,并探讨其与认知损伤之间的关系。方法共招募25例认知功能正常的MSA患者(MSA-normal cognition,MSA-NC),20例MSA-MCI患者和29例健康对照组。用FreeSurfer软件分析皮层厚度,并将皮层厚度的改变与临床及认知评分进行相关分析。结果与健康对照组相比,两个MSA亚组在额颞叶区域均表现出广泛的脑皮层形态学改变。MSA-MCI与MSA-NC组的比较显示左侧颞中回和颞下回的皮层厚度减低。左侧颞中回皮层厚度与认知评分和病程相关。结论MSA-MCI患者存在大脑结构的改变。左侧颞叶区域大脑皮层结构的改变可能是MSA-MCI患者认知功能下降的标志性改变。Objective This study aimed to investigate the morphometric alterations in the cortical and subcortical structures in multiple system atrophy(MSA)patients with mild cognitive impairment(MCI),and to explore the association with cognitive deficits.Methods A total of 25 MSA patients with normal cognitive function(MSA-NC),20 MSA patients with mild cognitive impairment(MSA-MCI)and 29 healthy controls were recruited.FreeSurfer software was used to analyse cortical thickness.Cortical thickness changes were correlated with cognitive scores.Results Compared to healthy controls,both MSA subgroups exhibited widespread morphology alterations of brain structures in the fronto-temporal regions.Direct comparison of MSA-MCI and MSA-NC patients showed cortical thinning in the left middle and inferior temporal gyrus(ITG)in MSA-MCI patients.Cortical thinning in the left middle temporal gyrus(MTG)correlated with cognitive assessment and disease duration.Conclusion Structural changes in the brain occur in MSA-MCI patients.The alteration of brain structure in the left temporal regions might be a biomarker of cognitive decline in MSA-MCI patients.
