摘  要:目的比较3D VIBE_(CAIPI-DIXON)和2D Flash_(GRAPPA)-fs在磁共振小肠造影(MRE)增强后图像中的应用价值。方法45例入组患者(男27例,女18例)使用3.0 T磁共振检查;同一患者在增强后分别进行冠状位屏气3D VIBE_(CAIPI-DIXON)及冠状位屏气2D Flash_(GRAPPA)-fs扫描,由两位医师分别进行图像质量评估,评价出质量较好的图像;同时进行半定量信号比,主要针对肠壁/内脏脂肪、肠壁/腹腔淋巴结信号比,并进行单因素方差分析。结果在所有图像中,两位医师共同认为3D VIBE_(CAIPI-DIXON)图像质量好的为82.2%;认为2D Flash_(GRAPPA)-fs图像质量好分别为2.2%和4.4%(医师1/医师2),进行计算Kappa值为0.779;这种优势的主要原因是3D VIBE_(CAIPI-DIXON)可以均匀的抑制脂肪,可减小运动伪影,更快的获取图像,从而使肠道壁显示的更加清晰。3D VIBE_(CAIPI-DIXON)肠壁/内脏脂肪信号比(9.3±3.3)和2D Flash_(GRAPPA)-fs肠壁/内脏脂肪信号比(3.3±1.0)具有显著差异性(P<0.05);3D VIBE_(CAIPI-DIXON)肠壁/淋巴结信号比(1.1±0.3)和2D Flash_(GRAPPA)-fs肠壁/淋巴结信号比(1.3±0.6)无显著差异性(P>0.05)。结论相较于传统的2D Flash_(GRAPPA)-fs,3D VIBE_(CAIPI-DIXON)能在更短时间内获得高质量的抑脂小肠图像,有助于屏气困难的患者获得高质量的图像。Objective To compare a fast 3 D VIBE sequence with Dixon fat saturation and CAIPIRINHA acceleration techniques(3 D VIBE_(CAIPI-DIXON))to a standard 2 D FLASH sequence with spectral fat saturation and conventional GRAPPA acceleration technique(2 D Flash_(GRAPPA-fs)for enhanced imaging of the small intestine.Methods In this retrospective,45 patients(18 female,27 male)examined on a 18-channel 3.0-T MR system were included;3 D VIBE_(CAIPI-DIXON) and 2 D Flash_(GRAPPA-fs) coronal sequences were performed in each subject in random order after the administration of an intravenous contrast agent.Two radiologists evaluated the images with regard to diagnostic preference;Semiquantitative signal ratios were calculated for the small intestine wall versus the visceral fat,and celiac lymph nodes.Signal ratio results were analyzed using a univariate analysis of variance.Results 3 D VIBE_(CAIPI-DIXON) was preferred in 82.2%(both readers)and 2 D Flash_(GRAPPA-fs) in 2.2%/4.4%(reader 1/2)of cases with a Kappa value of 0.779.The main reasons for this preference were homogenous fat saturation with 3 D VIBE_(CAIPI-DIXON) and reduced motion artifacts due to a faster acquisition,leading to improved delineation of the small intestine wall.Signal ratios of small intestine wall to fat signal for 3 D VIBE_(CAIPI-DIXON)(9.3±3.3)and 2 D Flash_(GRAPPA-fs)(3.3±1.0)were statistically different(P<0.05).However,no additional statistically significant differences in signal ratios were identified(range:1.1±0.3 to 1.3±0.6;P>0.05).Conclusion 3 D VIBE_(CAIPI-DIXON) can acquire small intestine imaging with a shorter time and improved fat suppression relative to conventional 2 D Flash_(GRAPPA-fs);3 D VIBE_(CAIPI-DIXON) can help patients who cannot hold their breath for a while to acquire a better image.
