摘　　要：目的:解读乙肝表面抗原(HBsAg)双试剂阳性而核酸检测呈阴性的报告,探究联合检测的意义。方法:选取2017年5月-2020年5月献血者标本内HBsAg血清学双试剂阳性的试样,均同时进行核酸检测,用原检测系统复测阴性标本。结果:共计采集22万份献血者检测标本。HBsAg血清学双试剂阳性标本92份,核酸阳性标本81份、阴性标本11份;其中核酸单检、混检阴性标本分别有4份、7份,对以上不同检测系统下的标本再行检查,至少有1次是阳性的标本分别有2份、5份。结论:不管是单检还是混检核酸系统,出现HBsAg血清学双试剂阳性,但核酸检测呈阴性的结果难以完全规避,应重视对核酸及血清学检测系统的测评,与实验室内原有检测法对比分析,规避漏检情况。Objective:To interpret the report that the double reagent of hepatitis B surface antigen(HBsAg)is positive but the nucleic acid test is negative,and to explore the significance of the combined test.Methods:From May 2017 to May 2020,the samples with HBs Ag serological double reagent positive in blood donors were selected,and perform nucleic acid testing at the same time,and retest negative samples with the original detection system.Results:A total of 220,000 blood donor test specimens were collected during the above time period.There are 92 HBs Ag serological double-reagent positive specimens,81 nucleic acid positive specimens,and 11 negative specimens.Among them,there are 4 and 7 negative specimens for single nucleic acid test and mixed test.The samples under the above different detection systems were reexamined,there were 2 specimens and 5 specimens that were positive at least once.Conclusion:Whether it is a single test or a mixed test nucleic acid system,HBs Ag serological double reagents are positive,but the result of a negative nucleic acid test is difficult to completely circumvent.The evaluation of nucleic acid and serological test systems should be paid attention to,compare and analyze with the initial testing methods in the laboratory to avoid missed testing.
