超声心动图诊断右肺动脉异常起源于主动脉的价值研究

摘　　要：目的:讨论超声心动图诊断右肺动脉异常起源于主动脉(AORPA)的价值。方法:选取2019年3月-2020年3月经超声心动图诊断的AORPA患者50例,观察心内结构、主动脉近端是否存在异常起源的血管,结合频谱多普勒和彩色多普勒观察肺动脉和主动脉之间的空间关系,并仔细观察肺动脉分支是否完整,观察超声心动图对于AORPA的诊断准确率,同时观察AORPA超声心动图的特点。结果:49例患者超声心电图诊断结果和手术诊断结果相符,出现1例漏诊为动脉导管未闭锁,近端型36例,远端型14例。近端型肺动脉收缩压(111.3±12.6)mm Hg,右肺动脉内径为(12.6±2.5)mm;远端型肺动脉收缩压(113.5±13.8)mm Hg,右肺动脉内径为(13.9±2.6)mm。超声心电图主要表现为肺动脉分叉结构消失,肺动脉主干延续于左肺动脉。结论:超声心动图在诊断AORPA中具有较高应用价值,可根据影像资料设计手术方案,利于患者治疗。Objective:To discuss the value of echocardiography in diagnosing abnormal right pulmonary artery originating from the aorta(AORPA).Methods:From March 2019 to March 2020,50 AORPA patients diagnosed by menstrual echocardiography were selected,we observed the structure of the heart and whether there are abnormal vessels in the proximal aorta.The spatial relationship between pulmonary artery and aorta was observed by spectral Doppler and color Doppler,and carefully observed whether the branches of the pulmonary artery are complete,observed the accuracy of the echocardiogram for the diagnosis of AORPA,and observed the characteristics of the AORPA echocardiogram.Results:The echocardiographic diagnosis results of 49 patients were consistent with the surgical diagnosis.One case was missed as an unoccluded arterial duct,36 cases were proximal type,and 14 cases were distal type.The systolic pressure of the proximal pulmonary artery was(111.3±12.6)mm Hg,and the inner diameter of the right pulmonary artery was(12.6±2.5)mm.The systolic pressure of the distal pulmonary artery was(113.5±13.8)mm Hg,and the inner diameter of the right pulmonary artery was(13.9±2.6)mm.The echocardiogram mainly showed that the bifurcation structure of the pulmonary artery disappeared,and the main pulmonary artery continued to the left pulmonary artery.Conclusion:Echocardiography has a high application value in the diagnosis of AORPA.The surgical plan can be designed based on the image data,which is beneficial to the treatment of patients.
