摘 要:目的:探究甲状腺超声与超声造影及细针穿刺在甲状腺结节良恶性诊断中的价值。方法:2019年1月-2020年4月收治甲状腺结节患者140例,所有患者均接受常规甲状腺超声、超声扫描和细针穿刺检查,以术后病理学结果为标准,比较三种方法对良恶性结节的诊断准确率、灵敏度及特异度。结果:140例患者中,良性结节110例,恶性结节30例;细针穿刺的诊断准确率、灵敏度及特异度均高于超声造影,而超声造影均高于甲状腺超声,差异均有统计学意义(P<0.05)。结论:甲状腺超声在甲状腺结节良恶性鉴别诊断中的应用价值不高,而超声造影和细针穿刺均有较高价值,其中细针穿刺的诊断价值高于超声造影,对于无条件开展细针穿刺的医院,可首选超声造影检查作为替代检查手段,可早期明确甲状腺结节的良恶性。Objective:To explore the value of thyroid ultrasound,contrast-enhanced ultrasound and fine needle aspiration in the diagnosis of benign and malignant thyroid nodules.Methods:From January 2019 to April 2020,140 patients with thyroid nodules were selected.All patients received routine thyroid ultrasound,ultrasound scanning and fine needle aspiration,based on the postoperative pathological results,we compared the accuracy,sensitivity and specificity of the three methods in the diagnosis of benign and malignant nodules.Results:Among 140 patients,110 cases were benign nodules and 30 cases were malignant nodules.The diagnostic accuracy,sensitivity and specificity of fine needle aspiration were higher than those of contrast-enhanced ultrasound,and contrast-enhanced ultrasound was higher than that of thyroid ultrasound,the difference was statistically significant(P<0.05).Conclusion:The application value of thyroid ultrasound in the differential diagnosis of benign and malignant thyroid nodules is not high,while contrast-enhanced ultrasound and fine-needle aspiration are of high value,the diagnostic value of fine-needle aspiration is higher than that of contrast-enhanced ultrasound,for the hospitals without conditions to carry out fine needle puncture,contrast-enhanced ultrasound can be the first choice as an alternative examination method,which can identify the benign and malignant thyroid nodules in the early stage.
