四维彩超联合超声弹性成像对宫颈占位性病变性质的诊断价值

摘　　要：目的分析四维彩超联合超声弹性成像(UE)对宫颈占位性病变性质的诊断价值。方法选取2019年1月至2020年12月项城市第二人民医院收治的100例宫颈占位性病变患者为研究对象,患者入院时均接受四维彩超、UE检查。以手术病理学检查结果为“金标准”,判定患者病变性质,比较四维彩超、UE单独及联合诊断与病理学检查结果的一致性、准确率、敏感度及特异度。结果病理学检查结果提示,100例宫颈占位性病变中良、恶性病变分别为41、59例。在对宫颈占位性病变性质的判定上,四维彩超与病理学检查结果一致性一般(Kappa=0.644,P<0.001),UE与病理学检查结果一致性一般(Kappa=0.702,P<0.001),四维彩超联合UE与病理学检查结果一致性较高(Kappa=0.874,P<0.001)。四维彩超联合UE诊断较四维彩超、UE单独诊断宫颈占位性病变性质的准确度、敏感度高(P<0.05)。结论四维彩超联合UE对宫颈占位性病变良恶性鉴别诊断价值较高。Objective To analyze the diagnostic value of four-dimensional color Doppler ultrasound combined with ultrasound elastography(UE)in the nature of cervical space-occupying lesions.Methods A total of 100 patients with cervical space-occupying lesions admitted to Xiangcheng Second People’s Hospital from January 2019 to December 2020 were selected as the research objects.All patients underwent four-dimensional color Doppler ultrasound and UE examination upon admission.The results of surgical pathological examinations were used as the“gold standard”to determine the nature of the patients’pathology.The consistency with the pathological examination,accuracy,sensitivity,and specificity of four-dimensional color Doppler ultrasound,UE alone and combined diagnosis were compared.Results The pathological examination showed that there were 41 cases of benign and 59 cases of malignant lesions in 100 cases of cervical space-occupying lesions.In judging the nature of cervical space-occupying lesions,four-dimensional color Doppler ultrasound and pathological examination results were generally consistent(Kappa=0.644,P<0.001),UE and pathological examination results were generally consistent(Kappa=0.702,P<0.001),and four-dimensional color Doppler ultrasound combined with UE and pathological examination results were consistent(Kappa=0.874,P<0.001).The four-dimensional color Doppler ultrasound combined with UE diagnosis was more accurate and sensitive than the two alone in diagnosing the nature of cervical space-occupying lesions(P<0.05).Conclusion Four-dimensional color Doppler ultrasound combined with UE is of high value in the differential diagnosis of benign and malignant cervical lesions.
