摘  要:目的分析多层螺旋CT血管造影(MSCTA)联合三维重建对肺内占位性病变良恶性的鉴别诊断价值。方法选取2018年12月至2020年12月商丘市中心医院收治的80例肺内占位性病变患者为研究对象,患者入院后均接受MSCTA检查,并将检查的图像进行三维重建。检查后行组织活检或手术病理检查,比较良恶性病变患者MSCTA三维重建参数,分析MSCTA联合三维重建对肺内占位性病变良恶性的诊断价值。结果病理检查显示,80例肺内占位性病变患者中52例(65.00%)为恶性病变,28例(35.00%)为良性病变。恶性病变组重建支气管动脉数多于良性病变组(P<0.05),支气管动脉内径大于良性病变组(P<0.05)。受试者工作特征曲线(ROC)显示,重建支气管动脉数、左侧支气管动脉内径、右侧支气管动脉内径单独及联合评估肺部恶性病变的曲线下面积(AUC)分别为0.822、0.796、0.789、0.866,均有一定诊断价值,联合诊断价值最高。结论MSCTA联合三维重建可显示肺部供血动脉,有效鉴别肺内良恶性占位性病变。Objective To analyze the value of multi-slice spiral CT angiography(MSCTA)combined with three-dimensional reconstruction in the differential diagnosis of benign and malignant pulmonary space-occupying lesions.Methods A total of 80 patients with pulmonary space-occupying lesions admitted to Shangqiu Central Hospital from December 2018 to December 2020 were selected as the research objects.After admission,all patients were examined by MSCTA,and the examination images were reconstructed in three dimensions.After the examination,tissue biopsy or surgical pathological examination was performed.The three-dimensional reconstruction parameters of MSCTA in patients with benign and malignant lesions were compared,and the diagnostic value of MSCTA combined with three-dimensional reconstruction in the diagnosis of benign and malignant pulmonary space-occupying lesions was analyzed.Results Pathological examination showed that 52(65.00%)of 80 patients with space-occupying lung lesions were malignant lesions and 28(35.00%)were benign lesions.More bronchial arteries were reconstructed in the malignant disease group than in the benign disease group(P<0.05),and the inner diameter of the bronchial artery was larger than that in the benign disease group(P<0.05).The receiver operating characteristic curve(ROC)showed that the area under the curve(AUC)of the number of reconstructed bronchial arteries,the diameter of left bronchial artery and the diameter of right bronchial artery alone or in combination in evaluating of the space-occupying lesion in lung were 0.822,0.796,0.789,and 0.866,respectively.They all had a certain diagnostic value,and the combined diagnostic value was the highest.Conclusion MSCTA combined with three-dimensional reconstruction could show the blood supply arteries of the lungs,and effectively distinguish benign and malignant space-occupying lesions in the lung.
