血清IL-10、sICAM-1水平与重症肺炎患者病情严重程度的相关性

摘　　要：目的探讨血清白细胞介素-10(IL-(-1)0)、可溶性细胞间黏附分子1(sICAM-1)水平与重症肺炎患者病情严重程度的相关性。方法选取2020年3月至2021年3月商丘市第四人民医院收治的80例重症肺炎患者作为研究对象,参照肺炎严重指数(PSI)分级分为低危组(27例)、中危组(36例)和高危组(17例)。收集患者的一般资料,检测血清IL-(-1)0、sICAM-1水平,分析血清IL-(-1)0、sICAM-1水平与重症肺炎患者病情严重程度的相关性。结果高危组和中危组血清IL-(-1)0、sICAM-1水平高于低危组,差异有统计学意义(P<0.05);中危组和高危组血清IL-(-1)0、sICAM-1水平比较,差异无统计学意义(P>0.05)。logistic回归分析显示,血清IL-(-1)0、sICAM-1过表达与重症肺炎患者病情严重程度有关(OR>1,P<0.05);绘制受试者工作特征曲线(ROC)显示,血清IL-(-1)0、sICAM-1单独及联合检测评估重症肺炎患者中/高危病情的曲线下面积(AUC)均>0.80,有一定评估价值,且联合检测价值最高。结论血清IL-(-1)0、sICAM-1水平与重症肺炎患者病情严重程度密切相关,临床可通过检测患者血清IL-(-1)0、sICAM-1水平评估患者病情严重程度。Objective To investigate the correlation between the serum levels of interleukin-10(IL-(-1)0)and soluble intercellular adhesion molecule-1(sICAM-1)and the severity of severe pneumonia in patients.Methods A total of 80 patients with severe pneumonia treated in Shangqiu Fourth People’s Hospital from March 2020 to March 2021 were selected as the research objects.According to the pneumonia severity index(PSI),they were divided into low-risk group(27 cases),medium-risk group(36 cases)and high-risk group(17 cases).The general data of patients were collected,the levels of serum IL-(-1)0 and sICAM-1 were detected,and the correlation between the levels of serum IL-(-1)0 and sICAM-1 and the severity of severe pneumonia was analyzed.Results The levels of serum IL-(-1)0 and sICAM-1 in high-risk group and medium-risk group were higher than those in low-risk group(P<0.05);There was no statistical significance in the levels of serum IL-(-1)0 and sICAM-1 between moderate-risk group and high-risk group(P>0.05).Logistic regression analysis showed that the overexpression of serum IL-(-1)0 and sICAM-1 was related to the severity of severe pneumonia(OR>1,P<0.05);Drawing the receiver operating characteristic(ROC)curve shows that the area under the curve(AUC)of serum IL-(-1)0 and sICAM-1 in evaluating the medium/high-risk condition of patients with severe pneumonia by single and combined examination was>0.80,which had certain evaluation value,and the value of combined examination was the highest.Conclusion The levels of serum IL-(-1)0 and sICAM-1 were closely related to the severity of patients with severe pneumonia.The severity of patients can be evaluated by detecting the levels of serum IL-(-1)0 and sICAM-1.
