摘  要:目的分析血清淀粉样蛋白A(SAA)、人软骨糖蛋白-39(HC-gp39)及铁蛋白(SF)检测在小儿难治性肺炎支原体肺炎(RMPP)预后评估中的应用价值。方法选取2018年3月—2020年4月本院收治的206例RMPP患儿作为RMPP组,根据病情程度又分为轻症亚组121例和重症亚组85例;另选取同期于本院接受健康体检结果正常的100例小儿作为对照组。对比RMPP组与对照组血清SAA、HC-gp39及SF水平差异,分析血清SAA、HC-gp39及SF在不同病情程度RMPP患儿中的表达情况,采用多因素Logistic回归分析探讨RMPP患儿预后的影响因素。结果RMPP组血清SAA、HC-gp39及SF水平较对照组高(P<0.01)。轻症亚组血清SAA、HC-gp39及SF水平较重症亚组低(P<0.01)。多因素Logistic回归分析显示,先天性心脏病、先天性气道发育异常、多系统并发症及SAA、HC-gp39、SF水平异常是影响RMPP患儿预后的独立危险因素(P<0.01)。结论血清SAA、HC-gp39及SF水平在RMPP患儿中显著上升,且与患儿预后密切相关,三者联合检测可作为评估患儿预后的有效手段。Objective To analyze values of serum amyloid A protein(SAA),human cartilage glycoprotein-39(HC-gp39)and ferritin(SF)in prognosis evaluation of children with refractory mycoplasma pneumoniae pneumonia(RMPP).Methods A total of 206 children with RMPP admitted between March 2018 and April 2020 were selected as RMPP group,and the patients were divided into mild subgroup(n=121)and severe subgroup(n=85)according to the severity of illness.In addition,100 healthy children who had undergone physical examinations at the same period were selected as control group.Differences of SAA,HC-gp39 and SF levels were compared between RMPP and control groups,and expressions of SAA,HC-gp39 and SF in RMPP children with different degrees of severity were analyzed.Influencing factors of prognoses of children with RMPP were analyzed by multivariate logistic regression analysis.Results Levels of SAA,HC-gp39 and SF in RMPP group were significantly higher than those in control group(P<0.01).Levels of serum SAA,HC-gp39 and SF in mild subgroup were significantly lower than those in severe subgroup(P<0.01).Multivariate logistic analysis showed that complicated congenital heart disease,congenital airway dysplasia,multisystem complications and abnormal levels of SAA,HC-gp39 and SF were independent risk factors affecting prognoses of children with RMPP(P<0.01).Conclusion Serum SAA,HC-gp39 and SF levels are significantly increased in children with RMPP,and they are closely related to prognoses of children.The combined detection of the three indexes can be used as an effective method to evaluate prognoses of children.
