结肠镜下高频电凝电切术与冷圈套切除术治疗结肠微小息肉的临床效果观察

摘　　要：目的分析结肠镜下高频电凝电切术与冷圈套切除术治疗结肠微小息肉的临床效果。方法回顾性分析2017年9月至2018年9月在我院行结肠微小息肉切除术的60例患者的临床资料,根据发现息肉后治疗方法的不同,分为高频电凝电切组和冷圈套切除组,每组各30例。记录两组切除息肉数目,比较两组息肉一般情况、切除情况,两组患者术后并发症及不良反应发生情况,术后2年息肉复发情况。结果冷圈套切除组切除息肉52枚,高频电凝电切组相应为56枚。两组息肉大小、部位、大体形态、标本回收率比较差异均无统计学意义(均P>0.05)。冷圈套切除组息肉切除时间短于高频电凝电切组,息肉完整切除率高于高频电凝电切组(均P<0.05)。两组患者术后并发症、不良反应发生率比较差异均无统计学意义(均P>0.05),冷圈套切除组术后2年息肉复发率低于高频电凝电切组(P<0.05)。结论与高频电凝电切术相比,采用冷圈套切除术治疗结肠微小息肉有助于提高息肉完整切除率,缩短息肉切除时间,降低术后息肉复发率。Objectives To compare clinical effectiveness of high-frequency electroblation and cold snare polypectomy for endo⁃scopic removal of colon micropolyps.Methods This was a retrospective analysis of 60 patients who underwent resection of co⁃lon micropolyps at our hospital between September 2017 and September 2018.Once polyps were identified under colonoscopy in these patients,the polyps were removed using high-frequency electroblation(30 patients,high-frequency electroblation group)or cold snare polypectomy(30 patients,cold snare polypectomy group).The number of polyps resected was recorded.The characteris⁃tics of the polyps,resection status,complications,adverse reactions,and polyps recurrence at 2 years after surgery were com⁃pared between the two groups.Results Fifty-two polyps were resected in the cold snare polypectomy group,and 56 were resect⁃ed in the high-frequency electroblation group.The size,location,gross appearance of the polyps,and specimen recovery rate were comparable between the two groups(P>0.05).The resection process was shorter in the cold snare polypectomy group,while the complete resection rate was higher in the cold snare polypectomy group(P<0.05).The incidence of postoperative complica⁃tions and adverse reactions were comparable between the two groups(P>0.05).The incidence of recurrence at 2 years was lower in the cold snare polypectomy group than in the high-frequency electroblation group(P<0.05).Conclusion Compared with highfrequency electroblation,cold snare polypectomy can increase complete resection rate,shorten theresection process,and lower re⁃currence risk for endoscopic removal of colon micropolyps.
