医师-药师协作的华法林抗凝门诊管理模式的应用及其效果评价

摘　　要：目的采用医师-药师协作的华法林抗凝门诊管理模式并对其效果进行评价。方法纳入2018年3月~2019年2月间我院门诊接受华法林抗凝治疗的患者,按先后顺序依次交叉分为研究组和对照组,比较两组患者抗凝治疗效果及不良事件。结果最终纳入分析131例,研究组65例,对照组66例。研究组与对照组患者基本信息差异无统计学意义(P>0.05)。研究组患者国际标准化比值(INR)达标率(56.79%vs. 40.24%,P=0.000)、INR稳定性(35.38%vs. 18.18%,P=0.026)及服药依从性评分[(7.42±0.55)分vs.(7.19±0.60)分,P=0.023]均高于对照组,差异有统计学意义。研究组出血不良事件发生比率为4.62%,低于对照组7.58%。结论应用医师-药师协作的华法林抗凝管理模式可提高患者华法林抗凝治疗质量、用药依从性,减少出血不良事件。OBJECTIVE To evaluate the effectiveness of warfarin anticoagulant management of outpatient by physicians in collaboration with pharmacists. METHODS The patients who received warfarin anticoagulant therapy in outpatient from March 2018 to February 2019 were divided into study group and control group in sequence.The anticoagulant effect and adverse events between the two groups were compared. RESULTS Finally, 131 cases were included in the study with 65 cases in the study group and 66 cases in the control group.There was no statistical difference in the patients’ basic information between the two groups.The international normalized ratio(INR)(56.79% vs. 40.24%, P=0.000), INR stability(35.38% vs. 18.18%, P=0.026) and medication compliance score [(7.42±0.55)vs.(7.19±0.60), P=0.023]of study group were significantly higher than that of the control group, and the difference was statistically significant.The bleeding adverse events in the study group(4.62%)was lower than that in the control group(7.58%). CONCLUSION The application of warfarin anticoagulant management by physicians in collaboration with doctors and pharmacists can improve the quality of warfarin anticoagulant treatment, medication compliance, and reduce bleeding adverse events.
