血清金属蛋白酶组织抑制因子、白细胞介素2与白细胞介素10和D-二聚体对下肢静脉曲张的诊断价值

摘　　要：目的探讨血清金属蛋白酶组织抑制因子(TIMP-1)、白细胞介素2(IL-2)、白细胞介素10(IL-10)、D-二聚体(D-dimer)对下肢静脉曲张的诊断价值。方法选取2019年1月至2020年6月无锡市第九人民医院收治的120例下肢静脉曲张患者为下肢静脉曲张组,另选取40例同院进行健康体检者作为对照组,检测两组血清TIMP-1、IL-2、IL-10、D-dimer含量,分析其在下肢静脉曲张诊断中的应用价值。结果下肢静脉曲张组患者血清TIMP-1、D-dimer、IL-2含量高于对照组,IL-10含量低于对照组(P<0.05);血清TIMP-1、IL-2、IL-10、D-dimer联合检测诊断下肢静脉曲张ROC曲线下面积为0.862高于单独检测ROC曲线下面积0.711、0.676、0.736、0.759(P<0.05);120例下肢静脉曲张患者洪堡静脉曲张严重程度评分量表(HVVSS)为(9.25±1.91)分,Pearson相关性分析结果显示患者HVVSS症状评分与血清TIMP-1、D-dimer呈正相关(r=0.358、r=0.321,P值均<0.05),与血清IL-2、IL-10含量无相关性(P>0.05)。结论下肢静脉曲张患者血清TIMP-1、D-dimer、IL-2和IL-10血清指标可作为下肢静脉曲张患者筛查指标,血清TIMP-1、D-dimer可反映患者疾病症状严重程度。Objective To explore the diagnostic value of serum tissue inhibitor of matrix metalloproteinase-1(TIMP-1),interleukin 2(IL-2),interleukin 10(IL-10)and D-dimer for lower extremity varicose veins(LEVV).Methods A total of 120 LEVV patients who were admitted to in Wuxi Ninth People′s Hospital from January 2019 to June 2020 were enrolled as LEVV group,while another 40 healthy people who underwent physical examination in the same hospital were enrolled as control group.The contents of serum TIMP-1,IL-2,IL-10 and D-dimer in both groups were detected.The diagnostic value in LEVV patients was analyzed.Results The contents of serum TIMP-1,D-dimer and IL-2 in LEVV group were higher than those in control group,while IL-10 content was lower than that in control group(P<0.05).The area under the ROC curve(AUC)of serum TIMP-1 combined with IL-2,IL-10 and D-dimer for diagnosis of LEVV was 0.862,higher than that of single index(0.711,0.676,0.736,0.759),P<0.05.HVVSS symptom score of the 120 LEVV patients was(9.25±1.91)points.The results of pearson correlation analysis showed that HVVSS symptom score was positively correlated with serum TIMP-1 and D-dimer(r=0.358,r=0.321,P<0.05),while not correlated with serum IL-2 and IL-10(P>0.05).Conclusions The contents of serum TIMP-1,D-dimer,IL-2 and IL-10 content decreases in LEVV patients,which can be applied to screen the patients.Serum contents of TIMP-1 and D-dimer can reflect the severity of diseases.
