微点阵激光治疗老年女性压力性尿失禁的效果

摘　　要：目的探讨微点阵激光治疗老年女性压力性尿失禁的效果。方法210例老年女性轻中度压力性尿失禁患者分为研究组107例,对照组103例,分别用微点阵二氧化碳激光治疗和生物反馈电刺激联合盆底肌肉锻炼法治疗3个月并随访6个月,分别在治疗前后、随访6个月时观察多个指标如盆底肌电位、阴道收缩力、尿失禁症状评分、生活质量评分及抑郁焦虑评分的变化。结果研究组治疗后的快速收缩期、持续收缩期及耐受测试期的盆底表面肌电位较对照组治疗后显著增高(P<0.05)。研究组治疗后漏尿程度改善占比82.24%(88/107);对照组治疗后漏尿程度改善占比60.19%(62/103);研究组治疗后改善率较对照组明显升高,差异有统计学意义(P<0.05)。研究组治疗后阴道收缩力较对照组治疗后显著升高,差异有统计学意义(P<0.05)。研究组治疗后、随访6个月的症状评分均显著低于同时期的对照组,研究组治疗后、随访6个月的生活质量评分均显著高于同时期的对照组,差异有统计学意义(P<0.05)。研究组治疗后、随访6个月的老年抑郁量表(GDS)评分和焦虑自评量表(SAS)评分均显著低于同时期的对照组,差异有统计学意义(P<0.05)。结论微点阵激光治疗轻中度老年女性压力性尿失禁具有良好的近期疗效,显著改善老年女性的生活质量和心理健康。Objective To explore the effect of fractional microablative laser treatment of stress urinary incontinence in elderly women.Methods Two hundred and ten elderly female patients suffering from light or moderate stress urinary incontinence randomly received different therapy such as fractional micro ablative laser treatment or biofeedback electrical stimulation plus pelvic floor muscle training for three months and were followed up for six months after therapy.Such index as surface electromyography value of pelvic floor muscles,vaginal contraction,symptoms of urinary in continence and quality of life of these patients were individually assessed.All data were statistically analyzed with Square-Test,T-Test and Wilcoxon test.Results The mean surface electromyography value of pelvic floor muscle after the therapy in test group was significantly higher than that of control group.The improvement rate of the test group after the therapy was 82.24%(88/107).The improvement rate of the control group after the therapy was 60.19%(62/103).The improvement rate of the test group after the therapy was statistically higher than that of control group(P<0.05).The vaginal contraction of the test group after the therapy were significantly higher than those in control group(P<0.05).The mean scores of symptoms of the test group after the therapy and after the six-month-follow-up were significantly lower than control group(P<0.05).The mean scores of quality of life of the test group after the therapy and after the six-month-follow-up were significantly higher than that of control group(P<0.05).The scores of GDS and SAS of the test group after the therapy and after the six-month-follow-up were significantly lower than control group(P<0.05).Conclusions The fractional micro ablative laser treatment evidently has a better effect for elderly women with urinary incontinence and improves their life quality and psychological health to a large extent.
