阿莫西林克拉维酸钾联合布地奈德治疗新生儿肺炎的效果及对中性粒细胞表面CD64、白介素-6的作用分析

摘　　要：目的探讨及分析阿莫西林克拉维酸钾联合布地奈德治疗新生儿肺炎的效果及对中性粒细胞表面CD64、白介素(interleukin,IL)-6的作用。方法选择2019年7月1日至2020年6月30日于蚌埠市第一人民医院接受治疗的52例新生儿肺炎患儿,采用随机数表法分为联合组(n=27)和单药组(n=25)。单药组给予布地奈德治疗,联合组在单药组基础上联合阿莫西林克拉维酸钾治疗。比较两组临床疗效、中性粒细胞表面CD64、IL-6、C-反应蛋白(C-reactive protein,CRP)、IL-4、IL-10、临床症状改善时间变化及不良反应发生情况。结果治疗后,联合组总有效率为96.30%,较单药组的76.00%高,差异具有统计学意义(P<0.05)。治疗前,联合组及单药组中性粒细胞表面CD64、IL-6比较无显著差异(P>0.05);治疗后,联合组和单药组中性粒细胞表面CD64、IL-6水平均随时间推移而降低,且联合组低于单药组,差异具有统计学意义(P<0.05)。治疗前,联合组和单药组CRP、IL-4、IL-10比较无显著差异;治疗后,联合组和单药组CRP、IL-4水平均随时间推移而降低,且联合组低于单药组,IL-10随时间推移而升高,且联合组高于单药组,差异具有统计学意义(P<0.05)。治疗后,联合组临床症状改善时间均低于单药组(P<0.05)。联合组和单药组不良反应发生情况比较无差异(P>0.05)。结论在新生儿肺炎患儿中应用阿莫西林克拉维酸钾联合布地奈德效果显著,可有效改善患儿中性粒细胞表面CD64、IL-6水平。Objective To investigate and analyze the effects of amoxicillin clavulanate potassium combined with budesonide in the treatment of neonatal pneumonia and the effects on neutrophil CD64 and interleukin(IL)-6.Methods A total of 52 neonates with pneumonia in our hospital from July 2019 to June 2020 were randomly divided into combination group(n=27) and monotherapy group(n=25).The monotherapy group was treated with budesonide,and the combination group was treated with amoxicillin and clavulanate potassium on the basis of the single drug group.The clinical efficacy,neutrophil CD64,IL-6,C-reactive protein(CRP),IL-4,IL-10,clinical symptom improvement time and adverse reactions were compared between the two groups.Results After treatment,the total effective rate of the combination group was 96.30%,which was higher than 76.00% of the monotherapy group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in CD64 and IL-6 of neutrophils between the combination group and the monotherapy group(P>0.05).After treatment,the levels of neutrophil CD64 and IL-6 in combination group and monotherapy group decreased over time,and the combination group was lower than monotherapy group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in CRP,IL-4 and IL-10 between the combination group and the monotherapy group.After treatment,the levels of CRP and IL-4 in combination group and monotherapy group decreased with time,and the combination group was lower than monotherapy group,while IL-10 increased with time,and the combination group was higher than monotherapy group,the difference was statistically significant(P<0.05).After treatment,the improvement time of clinical symptoms in the combination group was lower than that in the monotherapy group(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The application of amoxicillin and clavulanate potassium combined with budes
