活血补肾汤对椎体后凸成形术后骨质疏松性胸腰椎骨折患者的疗效观察

摘　　要：目的探讨活血补肾汤对椎体后凸成形术后骨质疏松性胸腰椎骨折患者的临床疗效。方法选取240例骨质疏松性胸腰椎骨折患者为研究对象,按随机数字表法将分为活血补肾组与常规组,各120例。常规组接受常规治疗,活血补肾组在常规组基础上接受活血补肾汤治疗,连续服用1个月。2组术后随访6个月。观察2组术后6个月的治疗效果,比较2组术前、术后第3天、6个月的疼痛、伤椎前缘高度、功能障碍恢复情况,术前、术后1、6个月的竖脊肌软组织张力位移情况以及术前、术后6个月的中国人骨质疏松症生存质量简明量表(COQOL)评分。结果术后6个月,活血补肾组总有效率(94.17%,113/120)高于常规组(84.17%,101/120)(P<0.05)。术前、术后第3天、6个月,2组视觉疼痛模拟评分(VAS)、Oswestry功能障碍指数(ODI)评分依次降低,且术后6个月,活血补肾组VAS、ODI评分低于常规组(P<0.05);2组伤椎前缘高度依次升高,且术后6个月,活血补肾组伤椎前缘高度高于常规组(P<0.05)。术前、术后1个月、6个月,2组竖脊肌软组织张力位移值依次升高,且术后1个月,活血补肾组竖脊肌软组织张力位移值高于常规组(P<0.05)。术后6个月,2组心理精神功能、疼痛症状、社会适应能力、身体功能评分低于术前,且活血补肾组低于常规组(P<0.05)。结论活血补肾汤可改善椎体后凸成形术后骨质疏松性胸腰椎骨折患者疼痛程度、竖脊肌软组织张力及功能障碍恢复情况,提高伤椎前缘高度,进而缓解患者的临床症状,提高疗效及生活质量。Objective To explore the effect of Huoxue Bushen Decoction on patients with osteoporotic thoracolumbar fractures after kyphoplasty.Methods A total of 240 patients with osteoporotic thoracolumbar fractures were selected as the research subjects,and randomly divided into a Huoxue Bushen group and a routine group,with 120 cases in each group.The routine group received a conventional treatment,while the Huoxue Bushen group was treated with Huoxue Bushen Decoction on the basis of the routine group,both for one month.The two groups were followed up for 6 months after operation.The treatment effect of the two groups was observed 6 months after operation.The pain,anterior edge height of the injured vertebrae,recovery of dysfunction in the two groups before operation,the 3rd day and the 6th month after operation were compared.The soft tissue tension displacement of erector spine muscle before operation,one and six months after operation and the scores of the concise Chinese osteoporosis quality of life scale(COQOL)before and six months after operation were compared between the two groups.Results Six months after operation,the total effective rate of the Huoxue Bushen group was 94.17%,higher than that of the routine group(84.17%)(P<0.05).Before operation,on the 3rd day and 6 months after operation,the scores of the visual pain analogue scale(VAS)and Oswestry dysfunction index(ODI)in the two groups were decreased sequentially,and 6 months after operation,both scores of the Huoxue Bushen group were lower than those of the routine group(P<0.05),while the anterior edge height of the injured vertebra in the two groups was increased sequentially,and 6 months after operation,the height in the Huoxue Bushen group was higher than that in the routine group(P<0.05).Before operation,one and six months after operation,the erector spinae soft tissue tension displacement values of the two groups increased sequentially,and one month after operation,the value of the Huoxue Bushen group was higher than that of the routine group(P<0.05).Six
