摘　　要：采用可视球囊阻断技术对2017年7月至2019年12月武汉大学中南医院26例大型或巨大型床突旁动脉瘤进行显微手术夹闭。26例动脉瘤均在术中成功阻断动脉瘤颈,在充分降低动脉瘤内张力后,成功塑形夹闭动脉瘤;术后即刻数字减影血管造影检查显示26个动脉瘤完全闭塞,无瘤颈残留、无载瘤动脉狭窄、无大血管栓塞事件发生。17例术前视力下降患者,术后视力改善13例。出院时改良Rankin量表(mRS)评分良好率为96.1%(25/26)。采用可视球囊阻断技术对复杂床突旁动脉瘤进行显微手术夹闭可降低手术难度,简化操作流程,是一种安全有效的复合手术方法。Twenty-six patients with large or giant paraclinoid aneurysms were treated with a visualized balloon occlusion-assisted technique in Zhongnan Hospital of Wuhan University from July 2017 to December 2019.All 26 patients were successfully treated with clip reconstruction after the aneurysm neck was blocked,and the aneurysm tension was fully reduced during the operation.Immediately after surgery,digital subtraction angiography showed that all lesions were completely excluded from the parent vessel with no residual aneurysm,and no parent artery stenosis or major embolism events occurred.Thirteen of 17 patients with previous visual deficits exhibited an improvement in visual acuity.The rate of good modified Rankin Scale(mRS)score after operation was 96.1%(25/26)at discharge.Microsurgical clipping of complicated large or giant paraclinoid aneurysms with the visualized balloon occlusion-assisted technique is a safe and effective compound surgical method to reduce the surgical difficulty and simplify the operation process.
