宫腔镜电切手术与刮宫术治疗子宫内膜息肉致异常子宫出血的疗效比较

摘　　要：目的比较宫腔镜电切手术与刮宫术治疗子宫内膜息肉致异常子宫出血的临床效果。方法选取本院2018年1月至2019年1月收治的子宫内膜息肉导致的异常子宫出血患者54例,按照入院顺序尾数的单双数分为宫腔镜组和常规组,每组27例。常规组实施刮宫术治疗,宫腔镜组实施宫腔镜电切手术治疗。比较两组患者手术时间、术中出血量、下床活动时间、住院时间、并发症发生率及疾病复发率。结果宫腔镜组手术时间、下床活动时间、住院时间均短于常规组,术中出血量少于常规组,差异有统计学意义(P<0.05);宫腔镜组并发症发生率低于常规组,差异有统计学意义(P<0.05);术后1年随访,宫腔镜组复发率低于常规组,差异有统计学意义(P<0.05)。结论宫腔镜电切手术治疗子宫内膜息肉导致的异常子宫出血可有效缩短患者手术时间和术后恢复时间,减少术后并发症发生率和疾病复发率,临床效果确切,临床应用价值较高,值得临床推广运用。Objective To compare the clinical effects of hysteroscopic electroresection and curettage in the treatment of abnormal uterine bleed-ing caused by endometrial polyps.Methods 54 patients with abnormal uterine bleeding caused by endometrial polyps admitted to our hospital from January 2018 to January 2019 were selected and divided into the hysteroscopic group and the conventional group according to the monotone and dou-ble numbers in the order of admission,with 27 cases in each group.The conventional group was treated with curettage,and the hysteroscopic group was treated with hysteroscopic electroresection.The operative time,intraoperative blood loss,time out of bed,hospital stay,complication rate and disease recurrence rate were compared between the two groups.Results The operation time,time out of bed and hospital stay in hysteroscopy group were shorter than those in conventional group,and the intraoperative blood loss was less than that in conventional group,the difference was statisti-cally significant(P<0.05).The complications in the hysteroscopic group were lower than those in the conventional group,and the difference was sta-tistically significant(P<0.05).The recurrence rate of the hysteroscopic group was lower than that of the conventional group,and the difference was statistically significant(P<0.05).Conclusion Hysteroscopic resection of abnormal uterine bleeding caused by endometrial polyps can effectively shorten the operation time and postoperative recovery time,reduce the incidence of postoperative complications and disease recurrence rate,with ac-curate clinical effect and high clinical application value,and is worthy of clinical promotion and application.
