带蒂复合组织瓣转移在拇指多指畸形矫形中的临床应用

摘　　要：目的采用从畸形拇指切除的指体切取带蒂复合组织瓣并转移,弥补保留指体的组织缺损矫正拇指多指畸形,以保证保留拇指良好的外观和功能。方法对保留拇指指体存在指体纤细短小,关节偏斜不稳定、指甲侧偏畸形、虎口狭窄及皮肤缺损等问题患儿20例,根据保留拇指指体组织缺损情况,从已切除的指体设计并切取带蒂皮瓣、指甲皮瓣及复合组织瓣进行局部转移修复。结果20例带蒂组织瓣均成活良好,重建拇指指体容积较大,关节稳定且具备一定的活动度,指甲较大,外形美观。结论从切除指体切取带蒂复合组织瓣进行局部转移,安全可靠,可有效弥补保留拇指指体的各种组织缺损,修复效果良好,值得临床推广应用。Objective To explore the pedicled composite tissue flap harvested from the severed thumb to cover the deficit of the saved one to achieve good appearance and favorable function.Methods In 20 patients with different defection,such as short and slender fingers,instable joint,nail deformity and skin defection,pedicel flap,nail flap or thumb-nail flap were harvested from the severed thumb acording to the deficit of the saved one to reconstruct the duplicated thumb.Results Twenty pedicled flaps survived successfully.Appropriate volume of the reconstructed thumbs with stable joint and good appearance were obtained.Conclusion It is a safe and reliable procedure to harvest a pedicled flap from the severed thumb.Defection of the preserved thumb,such as skin,nail and bone,can be restored by the flap effectively.
