摘  要:目的探讨近侧指间(Proximal Interphalangeal, PIP)关节损伤的3.0 T MRI表现。方法运用3.0 T磁共振成像仪、小柔线圈对2014年12月-2020年7月收治的21例PIP关节损伤患者进行MRI检查,分析PIP关节损伤的MRI表现特征。结果 21例PIP关节损伤共累及23个关节(单纯桡侧副韧带损伤9个关节,单纯掌板损伤2个关节,复合性肌腱韧带损伤共12个关节)。侧副韧带损伤MRI表现:侧副韧带增粗,T1WI呈低信号,FS-PDWI呈高信号,韧带连续性中断,周围软组织水肿。掌板损伤MRI表现:掌板T1WI及FS-PDWI信号升高,形态不规整,掌板连续性中断。中央束损伤MRI表现:中央束止点处FS-PDWI信号增高,中央束连续性中断。结论 3.0 T MRI检查是判断PIP关节损伤患者肌腱韧带损伤部位及损伤程度的有效检查方法,对诊断和治疗具有重要的临床应用价值。Objective To investigate the imaging features of the 3.0 T MRI in the PIP joint injuries.Methods Twenty-one cases of patient with PIP joint injury were studied with 3.0 T MRI and a small flex coil.Results Twenty-three joints were involved in 21 cases of PIP joint injury(including 9 joints with simple radial collateral ligament injury,2 joints with simple volar plate injury,12 joints with complex tendon and ligament injury).Collateral ligament injuries mainly manifested as thickening of the ligaments with low signal intensity on T1WI and high signal intensity on FS-PDWI,discontinuity of the ligaments and edema around the ligaments.Volar plate injuries mainly manifested as high signal intensity on T1WI and FS-PDWI,irregular shape,discontinuity of the volar plates.Central slip injuries were showed as high signal intensity at the insertion of tendons on FS-PDWI,discontinuity of the tendons.Conclusion 3.0 T MRI is an accurate method for evaluation of sites and degree of the ligament and tendon injuries in PIP injury.It is a useful tool for diagnosis and treatment.
