摘　　要：目的探讨配体白细胞介素17E(IL-17E)在结肠癌中的表达及其与肿瘤分期的相关性.方法选取100例不同分期的结肠癌患者为观察组,选取同期100例体检健康者作为对照组.两组均于入院当日检测血清IL-17E水平,比较结肠癌组、对照组的血清IL-17E水平,绘制受试者工作特征曲线(ROC),分析血清IL-17E对不同分期结肠癌的预测价值,获得最佳截断值.按照结肠癌TNM分期标准对结肠癌患者进行分期,根据ROC曲线得到的最佳截断值,将患者分为IL-17E高表达组、IL-17E低表达组,比较两组临床特征,分析血清IL-17E与临床特征的关系.采用Spearman相关性检验分析血清IL-17E与肿瘤分期的相关性.结果观察组患者血清IL-17E水平为(491.64±80.38)pg/mL,低于对照组的(789.52±206.75)pg/mL,差异具有统计学意义(P<0.05).ROC曲线提示:血清IL-17E预测结肠癌的曲线下面积为0.764(标准误=0.046,P<0.001,95%CI=0.673~0.855).IL-17E高表达组的肿瘤Ⅰ期、无淋巴结转移、肿瘤侵袭深度(T_(1)+T_(2))、高分化比例分别为50.00%、90.63%、93.75%、90.63%,高于IL-17E低表达组的17.65%、72.06%、67.65%、63.24%,差异具有统计学意义(P<0.05).Spearman相关性检验提示:血清IL-17E与肿瘤分期呈负相关(r=-0.523,P=0.013).结论与对照组比较,观察组患者血清IL-17E水平明显下调,且其与肿瘤分期呈负相关,肿瘤分期越高血清IL-17E水平越低.Objective To investigate the expression of ligand interleukin 17E(IL-17E)in colon cancer and its correlation with tumor staging.Method100 patients with colon cancer were selected as colon cancer group,and 100 patients who underwent physical examination during the same period were selected as control group.Serum IL-17E levels were measured on the day of admission in both groups.Serum IL-17E levels in colon cancer group and control group were compared.The predictive value of serum IL-17E for colon cancer was analyzed by ROC,and the best cut-off value was obtained.According to TNM staging criteria of colon cancer,colon cancer patients were divided into high expression group of IL-17E and low expression group of IL-17E according to the best cut-off value of ROC curve.The clinical characteristics of the two groups were compared and the relationship between serum IL-17E and clinical characteristics was analyzed.Spearman correlation test was used to analyze the correlation between serum IL-17E and tumor stage.ResultsThe serum level of IL-17E in colon cancer group was(491.64±80.38)pg/mL,lower than that in control group(789.52±206.75)pg/mL(P<0.05).ROC curve indicated that the area under curve of serum IL-17E predicting colon cancer was 0.764(standard error=0.046,P<0.001,95%CI=0.673~0.855).The proportion of stage I,non-lymph node metastasis,depth of invasion(T_(1)+T_(2))and high differentiation in IL-17E high expression group were 50.00%,90.63%,93.75%,90.63%,respectively,higher than that in IL-17E low expression group(17.65%,72.06%,67.65%,63.24%)(P<0.05).Spearman correlation test showed that serum IL-17E was negatively correlated with tumor stage(r=-0.523,P=0.013).ConclusionCompared with control group,the level of serum IL-17E in patients with colorectal cancer decreased significantly,and it was negatively correlated with the stage of cancer.The higher the stage of cancer,the lower the level of serum IL-17E.
