摘  要:目的探讨美沙酮维持治疗(MMT)对海洛因成瘾(HA)者大脑前岛叶功能连接的改变.方法招募26例HA、25例MMT者及42例正常对照(HC).对所有被试采集静息态功能磁共振成像(rsGfMRI)数据.以双侧前岛叶为感兴趣区(ROI)进行全脑的功能连接,统计分析MTT对HA大脑功能连接的影响.结果左侧岛叶与左侧眶额回、左侧中央后回及左侧颞中回功能连接存在显著差异(F=14.56,P<0.001;F=12.97,P<0.001;F=10.03,P<0.001;Alphasim校正),且MMT组显著低于HA组,趋于HC组;右侧前岛叶与双侧楔前叶功能连接存在显著性差异(F=11.38,P<0.001;F=9.79,P<0.001;Alphasim校正),且MMT组功能连接显著高于HA组与HC组.结论MMT增强HA大脑岛叶对多个脑区的功能连接,可能有利于岛叶对成瘾相关的动机驱动、躯体感觉与听觉功能做出积极监测与反应.Objective To explore the functional connection of the anterior insula cortex in heroin addiction(HA)patients with methadone maintenance treatment(MTT).Methods 26 HA,25 MMT and 42 health control(HC)were recruited.The resting-state functional MRI(rs-fMRI)data of all subjects were collected.The bilateral anterior insula cortex were used as the region of interest(ROI)for the functional connection of the whole brain.The difference of functional connection among the three groups was analyzed by ANOVA.Results There were significant differences in the functional connection between the left insula cortex and the left orbitofrontal gyrus,the left central posterior gyrus,and the left middle temporal gyrus(F=14.56,P<0.001;F=12.97,P<0.001;F=10.03,P<0.001;Alphasim correction),and the functional connection of MMT group was significantly lower than that of the HA group,tending to the HC group.There were significant differences in the functional connection between the right insular cortex and the bilateral precuneus cortex(F=11.38,P<0.001;F=9.79,P<0.001;Alphasim correction),and the functional connection of MMT group was significantly higher than that of HA group and HC group.Conclusion MMT enhances the functional connection of the insula cortex of HA to multiple brain regions,which may be beneficial to the insula cortex to actively monitor and respond to addiction-related motivational drives,somatosensory and auditory functions.
