摘 要:目的应用超微血管成像(SMI)技术评估颈动脉斑块内新生血管(IPN)与脑梗死的相关性。方法选取颈动脉斑块患者107例行颈动脉常规超声、SMI检查和颅脑MRI检查,分析不同回声IPN分布特征及SMI技术评估颈动脉IPN与脑梗死的相关性。结果混合回声IPN生成率明显高于等回声(P=0.030);颈动脉IPN的SMI评分与脑梗死显著相关(r=0.584,P=0.000);SMI评分1分、2分患者较0分患者更易发生脑梗死(P=0.000);SMI评分可预测颈动脉斑块患者脑梗死的发生[敏感性86.8%,特异性82.4%,曲线下面积(AUC)0.807,95%置信区间(CI)0.722~0.893](P=0.000)。结论颈动脉IPN的SMI评分与脑梗死的发生显著相关,可为脑梗死早期诊疗提供可靠依据。Objective To evaluate the correlation between carotid intraplaque neovascularization(IPN)and cerebral infarction by superb microvascular imaging(SMI)technique.Methods The study population consisted of 107 patients with carotid plaques.All of them received routine carotid ultrasound,SMI and cranial MRI.The distribution characteristics of IPN in different echo were analyzed,and the correlation between carotid IPN and cerebral infarction were evaluated by SMI.Results The IPN generation rate of mixed echo were significantly higher than that of iso-echoic(P=0.030);the SMI score of carotid IPN was significantly correlated with cerebral infarction(r=0.584,P=0.000);the patients with SMI score of 1 and 2 were more prone to cerebral infarction than patients with 0(P=0.000);SMI score was highly sensitivity(86.8%)and specificity(82.4%)to predict occurrence of the cerebral infarction[area under the curve(AUC)0.807,95% confidence interval(CI)0.772-0.893](P=0.000).Conclusion SMI score of carotid IPN is significantly related to cerebral infarction,and can provide a basic prediction for early diagnosis and treatment of cerebral infarction.
