摘 要:目的探讨多模态MRI技术在肛瘘联合分型中的临床应用价值.方法回顾性分析经手术证实的肛瘘患者36例,术前行常规MRI平扫、扩散加权成像(DWI)及肝脏快速容积采集(LAVA)序列,以上多种方法联合应用,将多模态MRI检查结果与手术结果对照.结果36例肛瘘用2种方法进行分型,并与手术分型对照,国内分类MRI符合率94.44%;国内分类与Morris分型联合应用符合率86.11%,其中应用联合分型诊断Morris1级、3级和Morris5级肛瘘准确度均达100%,诊断Morris2级、4级肛瘘准确度分别为83.33%、85.71%;这2种分型可以联合应用.结论多模态MRI技术能对肛瘘较准确分型;2种方法联合应用,能够更全面地评估肛瘘病变,为肛瘘手术提供重要的参考价值.Objective To investigate the clinical application value of multimodal MRI in the combined typing of anal fistula.Methods The clinical data of 36 patients were analyzed retrospectively,which were diagnosed as anal fistulas based on surgical pathology.Before operation,conventional plain MRI,diffusion weighted imaging(DWI)and liver acceleration volume acquisition(LAVA)sequences were scanned.The results of multimodal MRI were compared with the operation.Results Two different kinds of MRI typing were compared with the surgical typing respectively,then the coincidence rate of domestic MRI typing was 94.44%,and the coincidence rate of combined domestic and Morris typing was 86.11%.The accuracy of the combined typing in the diagnosis of Morris grade 1,3 and 5 anal fistula was 100%,and the accuracy in diagnosis of Morris grade 2 and 4 anal fistula was 83.33%and 85.71%respectively.These two kinds of typing could be used in combination.Conclusion Multimodal MRI can accurately classify anal fistula.The combination of the two kinds of typing can evaluate the anal fistula lesions more comprehensively,and provide guiding significance for the surgery.
