扩散张量成像在前交叉韧带重建术后及随访中的诊断价值

摘　　要：目的探讨扩散张量成像(DTI)在前交叉韧带重建术后及随访预后评估中的应用价值.方法选取膝关节DTI扫描的检查正常者100例和单侧前交叉韧带Ⅲ级、Ⅳ级损伤重建术者61例及随访者54例,在工作站划分前交叉韧带损伤、重建处及正常组相应部位的感兴趣区(ROI),测量ROI的各向异性分数(FA)值和表观扩散系数(ADC)值,同时生成纤维示踪图像.结果前交叉韧带损伤重建术前与手术后1周、3个月的FA值和ADC值有统计学差异(P<0.05);其中组间比较重建术后1周与3个月的FA值无统计学差异(P>0.05),ADC值有统计学差异(P<0.05).前交叉韧带损伤重建术后3个月和随访6个月、1年与正常组的FA值和ADC值有统计学差异(P<0.05);其中组间比较前交叉韧带损伤重建术后随访6个月与正常组的FA值和重建术后随访1年与正常组的ADC值无统计学差异(P>0.05),其他组间的FA值和ADC值有统计学差异(P<0.05).结论DTI的FA值和ADC值能够客观反映前交叉韧带损伤重建术后及随访中的微观结构变化,纤维示踪成像可以直观显示前交叉韧带重建处的三维形态,在前交叉韧带损伤重建术后及随访的预后评估中具有重要诊断价值.Objective To investigate the application value of diffusion tensor imaging(DTI)for prognosis evaluate after operation of the anterior cruciate ligament reconstruction and follow-up.Methods DTI scanning of 100 cases of normal knee joint,61 cases of patients with M grade and JV grade anterior cruciate ligament injury and 54 cases of patients with follow-up were selected.At the workstation,the regions of interest(ROI)of the anterior cruciate ligament injury,reconstruction and the corresponding position of normal group were divided,the fractional anisotropy(FA)values and apparent diffusion coefficient(ADC)values of the ROI were measured,and the tractography images were generated.Results The FA values and ADC values of the preoperative and the 1 week,3 months postoperative of anterior cruciate ligament reconstruction had statistically significant difference(P<0.05).There was no statistically significant difference in the FA values between the 1 week and 3 months postoperative of anterior cruciate ligament reconstruction(P>0.05),the ADC values was statistically significant difference(P<0.05).The FA values and ADC values of the 3 months and the 6 months,1 year in follow-up after operation of anterior cruciate ligament reconstruction and the normal group had statistically significant difference(P<0.05).There were no statistically significant differences in the FA values between the 6 months in follow-up after operation of anterior cruciate ligament reconstruction and the normal group,and the ADC values between the 1 year in follow-up after operation of anterior cruciate ligament reconstruction and the normal group(P>0.05).there were statistical significance differences in the FA values and ADC values among other groups(P<0.05).Conclusion The FA values and ADC values of DTI can objectively reflect the microstructural changes after operation of anterior cruciate ligament reconstruction and follow-up,the tractography imaging can visualize the three-dimensional morphology of anterior cruciate ligament reconstruction position
