血栓弹力图与常规凝血检查对患者凝血功能的相关性

摘　　要：目的分析血栓弹力图(TEG)与常规凝血检查临床患者凝血相关的相关性。方法选择2019年3月~2021年6月我院住院患者共100例。所有患者均行常规凝血检查和TEG检查。常规凝血检查指标包括活化部分凝血活酶时间(aPTT)、凝血酶原时间(PT)、纤维蛋白原(Fbg);TEG检查指标包括凝血反应时间(R)、血细胞凝集块形成时间(K)、血细胞凝集块形成速率(α角)、血凝块最大强度或硬度(MA)、凝血综合指数(CI)。采用Pearson相关性分析,比较常规凝血检查和TEG检查各指标的相关性。结果TEG检测的R值与常规凝血检测的PT呈正相关,与Fbg呈负相关。TEG检测的MA与常规凝血检测的Fbg显著正相关。TEG检测的CI与常规凝血检测的PT呈负相关,与Fbg呈正相关。TEG检测其余指标与常规凝血相关指标相关性不显著(P>0.05)。结论TEG和常规凝血检查部分指标存在一定的相关性。如将两种检测方法配合使用,可提高凝血功能检查的准确率。Objective Analysis of the correlation between Thromboelastography(TEG)and coagulation in clinical patients for routine clotting examination.Methods Selected a total of 100 inpatients from March 2019 to June 2021.All patients are subjected to routine clotting and TEG examinations.The regular coagulation test indicators include the active part of the clotting enzyme time(aPTT),the coagulase original time(PT),fibrinogen(Fbg);TEG inspection indicators include clotting reaction time(R),blood cell clotting block formation time(K),blood cell clotting block formation rate(αangle),maximum strength or hardness of blood clot(MA),and clotting composite index(CI).Using Pearson correlation analysis,the correlation between conventional coagulation examination and TEG examination was compared.Results The R value of TEG detection was positively related to PT of conventional coagulation test and negatively related to Fbg.The MA detected by TEG is significantly positively related to the Fbg of conventional clotting testing.The CI of TEG test was negatively related to PT of conventional coagulation test and positively related to Fbg.The remaining indicators of TEG detection were not significantly associated with conventional coagulation-related indicators(P>0.05).Conclusion There is a certain correlation between TEG and some indicators of conventional coagulation examination.If the two detection methods are used together,the accuracy of coagulation function can be improved.
