改良Bartlett手术治疗足拇趾嵌甲性甲沟炎的效果

摘　　要：目的探讨改良Bartlett手术治疗足拇趾嵌甲性甲沟炎的临床疗效。方法选取2020年1月-2021年1月我院收治的40例足拇趾嵌甲性甲沟炎患者作为研究对象,采用随机数字表法分为观察组和对照组,各20例。对照组采用常规拔甲术治疗,观察组采用改良Bartlett手术治疗,比较两组临床治疗疗效、术后恢复情况、感染发生率及复发率。结果观察组治愈率为95.00%,高于对照组的80.00%,但差异无统计学意义(P>0.05)。观察组干燥时间、创面愈合时间及VAS评分均低于对照组,差异有统计学意义(P<0.05),而两组换药次数比较,差异无统计学意义(P>0.05)。观察组感染率高于对照组,但差异无统计学意义(P>0.05)。观察组复发率低于对照组,差异有统计学意义(P<0.05)。结论改良Bartlett手术治疗足拇趾嵌甲性甲沟炎具有良好的临床疗效,能够有效缩短患者的痊愈时间,缓解疼痛情况,同时感染、复发风险较少,预后良好。Objective To explore the clinical effect of a modified Bartlett surgery on ingrown paronychia of great toe.Methods A total of 40 patients with ingrown paronychia of great toe in our hospital from January 2020 to January 2021 were selected as the research objects,and were randomly divided into observation group and control group with 20 cases in each group by digital table method.The control group was treated with conventional nail extraction,the observation group was treated with Bartlett surgery,the postoperative recovery,infection incidence and recurrence rate of the two groups were compared.Results The cure rate of observation group was 95.00%,which was higher than 80.00%of control group,but the difference was not statistically significant(P>0.05).The drying time,wound healing time and VAS score of the observation group were lower than those of the control group,the differences were statistically significant(P<0.05),while the times of dressing change between the two groups had no statistical significance(P>0.05).The infection rate of the observation group was higher than that of the control group,but the difference was not statistically significant(P>0.05).The recurrence rate of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion This modified Bartlett surgery has good clinical efficacy in the treatment of ingrown paronychia of great toe,which can effectively shorten the recovery time of patients,relieve pain,and reduce the risk of infection and recurrence,and have a good prognosis.
