血清RBC、FBG及RVED/LVED联合检测对急性肺栓塞预后价值

摘　　要：目的研究血清RBC、FBG及RVED/LVED联合检测对急性肺栓塞预后价值。方法选取2016年1月-2020年1月辽宁中医药大学附属第四医院收治的108例急性肺栓塞患者,根据患者出院时的存活和死亡情况分为死亡组(n=43)和存活组(n=65)。对比两组患者全球急性冠脉综合征注册研究(blobal registry of acute coronary syndrome,GRACE)评分、血清红细胞(erythrocyte/red blood cell,RBC)、纤维蛋白原(Fibrinogen FBG)、右室舒张末直径/左室舒张末直径比值(right ventricular end diastolic diameter/left ventricular end diastolic diame⁃ter,RVED/LVED);对GRACE评分与血清RBC、FBG、RVED/LVED进行相关性分析;对血清RBC、FBG、RVED/LVED进行相关预后预测价值分析。结果相比于存活组,死亡组GRACE评分显著更高(P<0.05);相比于存活组,死亡组RBC、FBG显著更高(P<0.05);相比于存活组,死亡组RVED、LVED、RVED/LVED显著更高(P<0.05)。经分析得出,GRACE评分与患者血清RBC、FBG和RVED/LVED呈正相关;经分析,血清RBC、FBG联合RVED/LVED检测的灵敏度和特异性均高于单独检测。结论血清RBC、FBG联合RVED/LVED预测急性肺栓塞患者预后价值较高,灵敏度和特异性均高于单独检测。Objective To study the prognostic value of combined detection of serum RBC,FBG and RVED/LVED in acute pulmonary embolism.Methods One hundred and eight patients with acute pulmonary embolism admitted to our hospital from January 2016 to January 2020 were selected and divided into death group(n=43)and survival group(n=65)based on the survival and the death of patients at discharge.The global Registry of acute coronary syn‐drome(GRACE)score,serum red blood cell(RBC),fibrinogen(FBG),right ventricular end diastolic diameter/left ventricular end diastolic diameter ratio(RVED/LVED)were compared between the two groups of patients.Correlation analysis was performed between GRACE score and serum RBC,FBG,RVED/LVED.The prognostic value of serum RBC,FBG,RVED/LVED was analyzed.Results Compared with the survival group,the GRACE score of the death group was significantly higher(P<0.05).Compared with the survival group,the RBC and the FBG were significantly higher in death group(P<0.05).Compared with the survival group,the death group had significantly higher RVED,LVED,and RVED/LVED(P<0.05).The analysis showed that the GRACE score was positively correlated with serum RBC,FBG and RVED/LVED;the sensitivity and the specificity of serum RBC and FBG combined with RVED/LVED were higher than those of single one.Conclusion The value of serum RBC,FBG combined with RVED/LVED in pre‐dicting the prognosis of patients with acute pulmonary embolism is high,and its sensitivity and specificity are both high‐er than single detection.
