中孕期胎儿单脐动脉的彩超特征分析

摘　　要：目的:分析中孕期胎儿单脐动脉(SUA)的彩超表现并总结漏诊及误诊原因。方法:选取发现显示为SUA胎儿的单胎妊娠孕妇48例,其中46例SUA胎儿接受中孕期彩超检查。结果:(1)中孕期共发现SUA胎儿48例,超声检查发现46例,其中1例误诊;2例畸形胎儿引产后发现为SUA胎儿。(2)SUA胎儿合并其它异常19例,占40%,其中8例心血管系统异常,1例骨骼系统畸形,3例泌尿系统异常,3例腹壁消化系统异常,3例中枢神经系统异常,1例多发系统畸形。(3)SUA胎儿合并其它异常中最常见的是心血管系统异常,占42.1%。(4)彩超检出45例SUA胎儿中,脐动脉血流频谱正常者36例,占80%;血流频谱异常者9例,占20%。(5)超声检出的SUA胎儿合并其它异常中有12例选择引产终止妊娠,余SUA胎儿33例自愿进行妊娠分娩,其中低体重儿2例,新生儿窒息1例,1例胎死宫内。结论:了解中孕期SUA胎儿彩超特征对减少漏诊及误诊有重要临床意义。Objective:To analyze the characteristics of fetal single umbilical artery(SUA)diagnosed by color ultrasonography during the middle pregnancy,and summarize the reasons for missed diagnosis and misdiagnosis.Methods:Forty-eight singleton pregnant women with SUA fetuses found were selected,of which 46 SUA fetuses received color ultrasound examination during middle pregnancy.Results:(1)A total of 48 cases of SUA were found in the middle pregnancy,46 cases were found by ultrasound examination,and 1 case was misdiagnosed.Two abnormal fetuses were found to be SUA fetuses after induction of labor.(2)19 cases of SUA fetuses with other abnormalities,accounting for 40%,of which 8 cases of cardiovascular system abnormalities,1 case of skeletal system abnormalities,3 cases of urinary system abnormalities,3 cases of abdominal wall digestive system abnormalities,3 cases of central nervous system abnormalities and 1 case of multiple occurrences system abnormalities.(3)Cardiovascular system abnormality was the most common abnormality in SUA fetuses with other abnormalities,accounting for 42.1%.(4)Of the 45 SUA fetuses detected by color ultrasound,36 cases(80%)had normal blood flow spectrum of umbilical artery.Abnormal blood flow spectrum was found in 9 cases(20%).(5)Among the SUA fetuses with other abnormalities detected by ultrasound,12 cases chose induced labor to terminate pregnancy,while the remaining 33 SUA fetuses volunteered to carry out pregnancy and delivery,including 2 cases of low birth weight,1 case of neonatal asphyxia,and 1 case of fetal death in utero.Conclusion:Understanding the characteristics of color ultrasonography in the diagnosis of SUA during the middle pregnancy and how to reduce the incidence of missed diagnosis and misdiagnosis.
