养阴和瘀通窍汤辅助治疗非痴呆型血管性认知功能障碍的疗效及对大脑及椎底动脉血流流速的影响

摘　　要：目的探讨非痴呆型血管性认知功能障碍给予养阴和瘀通窍汤辅助治疗的临床疗效及对大脑及椎底动脉血流流速的影响作用。方法选择2019年5月—2020年5月本院收治的72例非痴呆型血管性认知功能障碍患者作为研究对象,按随机数表法分为观察组和对照组两组,每组各36例。对照组给予尼莫地平治疗,观察组在此基础上加以养阴和瘀通窍汤治疗,比较两组临床疗效、精神和认知状态、大脑及椎底动脉血流流速、生活质量以及不良反应。结果观察组临床有效率为77.78%,高于对照组的63.89%,差异无统计学意义(P>0.05);观察组治疗后MMSE、MoCA评分和ACA、MCA、PCA、VA、BA流速均优于对照组,差异有统计学意义(P<0.05);治疗后两组心理功能、物质功能评分差异无统计学意义(P>0.05),观察组躯体功能、社会功能评分高于对照组,差异有统计学意义(P<0.05);两组均未出现不良反应。结论对非痴呆型血管性认知功障碍患者给予养阴和瘀通窍汤辅助治疗其临床效果明显,可有效改善患者精神及认知状态、大脑及椎底动脉血流流速,促进生活质量提高,且安全性好。Objective To investigate the clinical effect of Yangyin Huyu Tongqiao decoction in the adjuvant treatment of non-dementia type vascular cognitive dysfunction and its effect on cerebral and vertebral artery blood flow velocity.Methods From May 2019 to May 2020,72 patients with non-dementia type vascular cognitive dysfunction those were admitted to our hospital were selected as the research objects.According to random number method,they were divided into observation group and control group,with 36 cases in each group.The control group was treated with Nimodipine,and the observation group was treated with Yangyin Huyu Tongqiao decoction on the basis of this.The clinical efficacy,mental and cognitive status,cerebral and vertebral artery blood flow velocity,quality of life and adverse reactions were compared between the two groups.Results The clinical effective rate of the observation group was 77.78%,which was higher than that of the control group(63.89%),however,the difference was not statistically significant(P>0.05).After treatment,MMSE,MOCA scores and ACA,MCA,PCA,VA and BA flow rates in the observation group were better than those in the control group,with statistical significance(P<0.05).After treatment,there was no significant difference in the scores of psychological function and physical function between the two groups(P>0.05),but the scores of physical function and social function in the observation group were higher than those in the control group(P<0.05).There were no adverse reactions in the two groups.Conclusions The adjuvant treatment of Yangyin Huyu Tongqiao decoction for patients with non-dementia vascular cognitive dysfunction presents obvious clinical effect,which could effectively improve the mental and cognitive status of patients,cerebral and vertebral artery blood flow velocity,and promote the improvement of life quality,with good safety.
