摘　　要：目的评估骨质疏松椎体压缩骨折行分步体位复位后低剂量骨水泥球囊扩张椎体成形术临床疗效。方法选择2016年1月—2020年1月本院收治的46例骨质疏松椎体骨折患者作为研究对象,分步体位复位后行低剂量骨水泥球囊扩张椎体成形术治疗。根据骨水泥用量不同将患者随机分为小剂量组和常规剂量组两组,每组各23例。观察两组患者一般情况、手术前后VAS评分、术前术后Cobb角、椎体前缘高度、中线高度及相关并发症。结果小剂量组和常规剂量组术后患者VAS评分、Cobb角、前缘高度、中线高度差异均无统计学意义(P>0.05)。小剂量组骨水泥渗漏率、临近椎体骨折发生率、肺栓塞发生率明显低于常规剂量组。结论分步体位复位联合低剂量骨水泥球囊扩张椎体成形术治疗老年骨质疏松性脊柱压缩骨折疗效显著、操作简单、并发症发生率低、安全可靠、适合临床长期推广应用。Objective To evaluate the clinical effect of minute step body posture restoration combined with percutanous kyphoplasty using low dose bone cement in the treatment of osteoporotic vertebral compression fracture.Methods From January 2016 to January 2020,46 cases of patients with osteoporotic vertebral compression fracture those were admitted to our hospital were enrolled as study subjects.After minute step body posture restoration,low dose bone cement percutanous kyphoplasty was used for therapy.According to the amount of bone cement,the patients were divided into low dose group and routine dose group,23 cases in each group.The general situation of patients,VAS score before and after operation,preoperative and post-operative Cobb angle,anterior height of vertebral body,height of midline,and related complications of the two groups were observed.Results There was no significant difference in VAS score,Cobb angle,anterior margin height and midline height between low dose group and routine dose group(P>0.05).The leakage rate of cement,the incidence of adjacent vertebral fractures and the incidence of pulmonary embolism were significantly lower in the low dose group than those in the conventional dose group.Conclusions The treatment of senile osteoporotic spinal compression fracture with minute step body posture restoration combined with low dose cemented balloon dilatation vertebroplasty is effective,it is simple,with low complication rate,safe and reliable,suitable for clinical application for a long period.
