摘 要:目的探讨腹主动脉球囊预置术对凶险性前置胎盘患者剖宫产术中X射线透视时间及出血情况的影响。方法回顾性分析2018年1月至2020年9月在新乡市中心医院分娩的62例凶险性前置胎盘产妇资料。其中,32例剖宫产术前行髂内动脉球囊预置术的患者作为A组,30例剖宫产术前行腹主动脉球囊预置术的患者作为B组。比较两组球囊预置时间、透视时间、放射剂量和剖宫产手术时间、术中出血量及术后体温。结果B组球囊预置时间、透视时间短于A组,而放射剂量少于A组(P<0.05)。两组手术时间、术中出血量、术后体温比较,差异无统计学意义(P>0.05)。结论双侧髂动脉球囊预置术和腹主动脉球囊预置术均能减少凶险性前置胎盘患者剖宫产术中出血量,但腹主动脉球囊预置术能够显著减少X射线透视时间和放射剂量,其在母胎保护方面效果更为理想。【Objective】To investigate the effect of abdominal aortic saccule preplacement on time and bleeding of X-ray fluoroscopy during cesarean delivery in patients with pernicious placenta previa.【Methods】The data of 62 cases of maternal with pernicious placenta previa delivered in our hospital from January 2018 to September 2020 were retrospectively analyzed.Thirty-two patients who underwent the internal iliac aortic saccule preplacement before cesarean section were included in group A.Thirty patients who underwent abdominal aortic saccule preplacement before cesarean section were included in group B.Saccule preplacement time,fluoroscopy time,radiation dose,cesarean section time,intraoperative bleeding and postoperative body temperature were compared between the two groups.【Results】The saccule preplacement time and fluoroscopy time were shorter in group B than in group A,and the radiation dose was less in group B than in group A,the difference was statistically significant(P<0.05).The differences in operation time,intraoperative bleeding,and postoperative temperature between the two groups were not statistically significant(P>0.05).【Conclusion】Bilateral iliac aortic saccule preplacement and abdominal aortic saccule preplacement both reduce bleeding during cesarean section in patients with pernicious placenta previa.However,abdominal aortic saccule preplacement significantly reduces time and radiation dose of X-ray fluoroscopy and is more effective in maternal-fetal protection.
