不同就诊时间段的经皮注射黄体酮所致皮下脂膜炎超声声像图特征分析

摘　　要：目的探讨经皮注射黄体酮后不同时间段引起的皮下脂膜炎的超声声像图特征,以提高超声诊断水平。方法收集2014年7月~2020年7月在重庆医科大学附属大学城医院行黄体酮注射保胎治疗后产生脂膜炎反应的62例患者临床及超声资料进行回顾性分析,以分析其声像图与就诊时间的演变关系。将上述患者的脂膜炎超声声像图类型分为高回声为主的结节硬化型、以无回声为主的液化型以及以强回声为主的钙化型,并按照不同检查时间段进行分组(即将其分为A、B两组)(A组≤6个月,37例;B组>6个月,25例),就其声像图类型进行组内及组间比较。结果62例黄体酮注射后患脂膜炎患者中,6个月内就诊者声像图表现以高回声结节硬化型为主,6个月后就诊者声像图表现以无回声的液化型为主,以强回声为主的钙化型少见,两组内及组间比较差异均有统计学意义(P<0.05)。结论掌握这些声像图变化特征在诊治黄体酮注射所致的脂膜炎中具有一定的指导意义。Objective To investigate ultrasonographic characteristics in different periods of subcutaneous panniculitis caused by progesterone injection for improving ultrasonic diagnosis accuracy.Methods From July 2017 to July 2020,a total of 62 cases with panniculitis reaction after progesterone injection,who were recruited into Chongqing Medical University Town Hospital,were included into this study for examining the relationship of their ultrasonographics with time.We divided the 62 cases'clinical data into two groups by the time:group A of less than six months(n=37),and group B of more than six months(n=25),in which ultrasonography showed three types:the high-level echo type,the echoless type,and the strong-level echo and type.Results The ultrasonographic types of panniculitis reaction after progesterone injection differed in examination time,showing high-level echo in 6 months after progesterone injection,echoless after 6 months of progesterone injection.There were significant differences in both groups or between them(P<0.05).Conclusion Mastering these ultrasonographic characteristics is valuable for promptly diagnosis of subcutaneous panniculitis caused by progesterone injection,has a certain clinical guiding significance.
