Monaco与Pinnacle计划系统在胸中段食管癌调强放疗中的剂量学比较

摘　　要：目的对Monaco和Pinnacle两套计划系统在胸中段食管癌调强放疗计划中的剂量学进行比较。方法选取16例胸中段食管癌患者,分别利用Monaco 5.11和Pinnacle^(3)9.10两套计划系统做调强计划,比较两个计划的靶区剂量分布、适形度指数(CI)、均匀性指数(HI)及危及器官的受照剂量差异。结果Monaco计划靶区的适形度指数和均匀性指数均优于Pinnacle(P<0.05),D_(mean)低于Pinnacle计划(P<0.05),Monaco计划中左右肺的受照剂量V_(5),右肺的V_(20)和D_(mean)都高于Pinnacle计划(P<0.05),Monaco计划的心脏V_(30)和V_(40)低于Pinnacle计划(P<0.05),其他指标在两种计划中的差别无统计学意义。结论两种计划系统均能满足胸中段食管癌的调强放疗计划,Monaco在靶区剂量分布和心脏保护方面有优势,Pinnacle在肺的保护方面有优势。Objective To compare the dosimetric differences of Monaco and Pinnacle planning systems(TPS)intensity-modulated radiotherapy(IMRT)for middle thoracic esophageal cancer.Methods 16 patients with middle thoracic esophageal cancer were selected,the Monaco 5.11 TPS and Pinnacle^(3)9.10 TPS were respectively applied to design two IMRT plans for each patient,the dose distribution in target areas,conformity index(CI),homogeneity index(HI),and the dose-volume differences in organs-at-risk were Compared.Results The conformity index,homogeneity index were better than those in Pinnacle plan(P<0.05),The mean Dose of PTV in Monaco plan was lower than that in Pinnacle plan(P<0.05),the V_(5) of left and right lung,V_(20) and the mean dose of right lung in Monaco plan were higher than those in Pinnacle plan(P<0.05),the V_(30) and V_(40) of heart in Monaco plan were lower than those in Pinnacle plan(P<0.05),and other indexes had no significant difference between the two plans.Conclusion For middle thoracic esophageal cancer,both systems can meet the clinical needs.Monaco has advantages in target dose distribution and heart protection,while Pinnacle has advantages in lung protection.
