摘  要:目的:分析腔隙性脑梗死伴有认知障碍(LICI)患者磁共振成像(MRI)的表现。方法:选取医院收治的65例腔隙性脑梗死伴有LICI患者纳入观察组,另选63例腔隙性脑梗死无LICI患者纳入对照组,观察对比两组患者头颅MRI影像学差异。结果:观察组患者侧脑室旁、丘脑、额叶、脑左侧以及双侧部位占比率显著高于对照组,差异有统计学意义(χ^(2)=30.039,χ^(2)=20.189,χ^(2)=24.281,χ^(2)=30.806,χ^(2)=9.570;P<0.05);脑右侧梗死比率显著低于对照组,差异有统计学意义(χ^(2)=5.850,P<0.05);其他梗死部位比较,差异无统计学意义。观察组哈氏值为(59.9±6.36)mm,显著高于对照组的(53.27±7.47)mm,差异有统计学意义(t=5.424,P<0.05)。结论:LICI患者病灶数目、体积及部位存在明显异常,且脑萎缩的发生率会提高,该情况会加重患者LICI表现。LICI患者扣带、丘脑前辐射、钩状束、海马、内囊后肢、额枕下束和视辐射部位的白质微细结构影像表现呈完整性破坏情况。Objective:To analyze the manifestations of magnetic resonance imaging(MRI)of patients with lacunar infarction cognitive impairment(LICI).Methods:65 patients with LICI who admitted to hospital were selected into the observation group,and 63 patients with LI but without LICI were selected as the control group.The iconographical difference of cerebral MRI between two groups were observed.Results:The proportions of periventricular,thalamus,frontal lobe,left side of brain and bilateral parts of brain in observation group were significantly higher than those in control group(χ^(2)=30.039,χ^(2)=20.189,χ^(2)=24.281,χ^(2)=30.806,χ^(2)=9.570,P<0.05).The ratio of cerebral right infarction of observation group was significantly lower than that of control group(χ^(2)=5.850,P<0.05).There was no significant difference in other infarct sites between two groups.The Hartmann value of observation group was(59.9±6.36)mm,which was significantly higher than that of control group(53.27±7.47)mm,and the difference of that between two groups was significant(t=5.424,P<0.05).Conclusion:There are obvious abnormal in the number,volume and location of lesions in patients with LICI,and the incidence rate of cerebral atrophy will be increased,and this would aggravate the LICI appearance of patients.In the cingulate,anterior thalamic radiation,hook bundle,hippocampus,posterior limb of internal capsule,fronto occipital bundle and visual radiation in patients with LICI,the image manifestation of the microstructure of white matter showed integrity damage.
