摘 要:目的:探讨开业护士(NP)角色管理在达芬奇机器人辅助下肝胆胰外科手术患者围手术期的应用价值。方法:NP角色从管理、医疗、患者、护理和多学科5方面划分职责,围绕术前管理、术中配合和术后照护开展护理;达芬奇手术机器人辅助完成脏器及病灶的探查定位和手术的精细化操作,NP角色+达芬奇机器人辅助手术管理方法构成联合手术管理模式。选取医院肝胆胰外科接治的40例手术患者,采用随机数表法分为对照组和观察组,每组20例,对照组采用常规手术管理模式,观察组采用联合手术管理模式,对比两组围手术期、术后康复和心理干预指标的差异。结果:观察组患者围手术期手术时间、术中出血量、术后排气时间、下床时间和住院时间数据好于对照组,差异有统计学意义(t=2.104,t=2.743,t=2.111,t=2.724,t=2.332;P<0.05);患者术后并发症总例数少于对照组,差异有统计学意义(χ^(2)=8.533,P<0.05);患者干预后焦虑自评量表(SAS)和抑郁自评量表(SDS)评分低于对照组,差异有统计学意义(t=8.934,t=11.275;P<0.05)。结论:NP角色+达芬奇机器人手术管理模式可提升医护人员业务能力,改善诊疗活动中医护人员的协作水平。Objective:To explore the application value of Nurse Practitioner(NP)role management in the perioperative period of hepatobiliary and pancreatic assisted by da Vinci surgical robot.Methods:The roles of NPs were divided in terms of management,medical treatment,patients,nursing and multidisciplinary to carry out nursing care focusing on preoperative management,intraoperative cooperation and postoperative care.Da Vinci surgical robot assisted in the exploration and positioning of organs and lesions,as well as the meticulous operation.NP role plus Da Vinci robot assisted surgery management method constituted a joint surgery management model.40 patients with of hepatobiliary and pancreatic surgery were selected and divided into the control group and the observation group by random number table method,with 20 cases in each group.The control group adopted routine operation management mode;the observation group adopted combined operation management mode.The difference of perioperative period,postoperative rehabilitation and psychological intervention between the two groups was compared.Results:The scores of 5 indexes in the observation group during the perioperative period were better than those in the control group,and the difference was statistically significant(t=2.104,t=2.743,t=2.111,t=2.724,t=2.332;P<0.05);the total number of postoperative complications in patients was less than that in control group,and the difference was statistically significant(χ^(2)=8.533,P<0.05);the scores of Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)of the patients after intervention were lower than those of control group,and the difference was statistically significant(t=8.934,t=11.275;P<0.05).Conclusion:The joint surgery management model can enhance the professional competence of medical staff and improve the level of collaboration of medical staff in clinical diagnosis and treatment activities.
