腹部推拿联合足三里温和灸对AECOPD胃肠功能障碍的改善作用研究

摘　　要：目的回顾性评价腹部推拿联合足三里温和灸治疗慢性阻塞性肺疾病急性加重期(AECOPD)胃肠功能障碍的疗效以及对血清胃动素(MOT)和胃泌素(GAS)水平的影响。方法将76例AECOPD胃肠功能障碍患者分为对照组与治疗组各38例,两组患者均根据指南予常规干预措施,其中对照组予吗丁啉口服,治疗组在对照组治疗基础上予腹部推拿联合足三里温和灸。比较两组腹腔内压力、胃肠功能障碍评分、临床疗效以及血清MOT和GAS水平。结果治疗后,治疗组腹腔内压力及胃肠功能障碍评分显著低于对照组(P <0.01);治疗组总有效率显著高于对照组(P <0.05);治疗组血清MOT水平明显高于对照组,GAS水平明显低于对照组(P <0.01)。结论腹部推拿联合足三里温和灸可促进AECOPD胃肠功能障碍的改善,可能与其调节血清MOT和GAS水平有关。Objective:To retrospectively evaluate the efficacy of abdominal massage combined with mild moxibustion of Zusanli on the improvement of gastrointestinal dysfunction in cases with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) and its effect on serum levels of motilin(MOT) and gastrin(GAS).Methods:A total of 76 patients with AECOPD and gastrointestinal dysfunction in our hospital were divided into the control group and observation group according to random number table method,38 cases in each group. All cases were treated with the conventional interventions according to the guidelines;the control group was orally given with domperidone,and the observation group was treated with abdominal massage combined with mild moxibustion of Zusanli on the basis of control group. Intraperitoneal pressure,gastrointestinal dysfunction score,the clinical efficacy,and serum levels of MOT and GAS were compared between the two groups. Results:After treatment,the intraperitoneal pressure and the score of and gastrointestinal dysfunction in the observation group were significantly lower than those in the control group(P < 0.01). The total effective rate of the observation group was significantly higher than that of the control group(P < 0.05). After treatment,the serum level of MOT in the observation group was significantly higher,while the GAS was obviously lower than those in the control group(P < 0.01). Conclusion:Abdominal massage combined with mild moxibustion of Zusanli can promote the improvement of gastrointestinal dysfunction in cases with AECOPD,which may be related to regulation of serum levels of MOT and GAS.
