半夏白术天麻汤合桃红四物汤加味治疗后循环缺血眩晕(痰瘀内阻证)的疗效观察

摘　　要：目的观察半夏白术天麻汤合桃红四物汤加味治疗后循环缺血眩晕(痰瘀内阻证)的临床疗效并探讨其作用机制。方法患者80例随机分为两组各40例,对照组予倍他司汀片,观察组在对照组治疗基础上采取半夏白术天麻汤合桃红四物汤加味内服,两组疗程均为2周。比较两组眩晕评定量表(DARS)、眩晕障碍量表(DHI)、痰瘀内阻证证候评分以及临床疗效,运用多普勒超声检查双侧椎动脉和基底动脉的血流速度。结果治疗后两组DARS和DHI评分、痰瘀内阻证证候评分较治疗前均显著降低(P <0.01),且观察组显著低于对照组(P <0.01)。治疗后两组左椎动脉、右椎动脉、基底动脉的血流速度较治疗前均显著增加(P <0.01),且观察组显著高于对照组(P <0.01)。观察组总有效率为92.50%,高于对照组的72.50%(P <0.05)。结论半夏白术天麻汤合桃红四物汤加味能够减轻后循环缺血眩晕(痰瘀内阻证)患者的眩晕症状,改善中医症状,增加后循环血流,提高疗效。Objective:To investigate the clinical efficacy of Banxia Baizhu Tianma Decoction combined with Taohong Siwu Decoction in treating posterior circulation ischemic vertigo with syndrome of phlegm and blood stasis resistance.Methods:80 patients with posterior circulation ischemic vertigo were selected and divided into the observation group and control group by random number table method,40 patients in each group.The control group was given betastatin tablets,while the observation group took Banxia Baizhu Tianma Decoction combined with Taohong Siwu Decoction on the basis of the treatment in the control group.The course of treatment was 2 weeks in both groups.Scores of system of dizziness assessment rating scale(DARS),dizziness handicap inventory(DHI),and symptoms of syndrome of phlegm and blood stasis resistance and clinical efficacy of the two groups were compared.Cranial doppler ultrasound was used to examine the blood flow velocity of bilateral vertebral artery and basilar artery.Results:After treatment,scores of DARS,DHI,and symptoms of syndrome of phlegm and blood stasis resistance in the observation group were significantly lower than those in the control group(P<0.01).After treatment,the blood flow velocity of left vertebral artery,right vertebral artery,and basilar artery in the observation group were significantly higher than those of the control group(P<0.01).The total effective rate of the observation group was 92.50%,which was significantly higher than 72.50%of control group(P<0.05).Conclusion:Banxia Baizhu Tianma Decoction combined with Taohong Siwu Decoction can alleviate the vertigo symptoms of patients with posterior circulation ischemic vertigo with syndrome of phlegm and blood stasis resistance,improve the symptoms of traditional Chinese medicine,increase the circulating blood flow,and improve the curative effect.
