摘  要:目的比较普伐他汀灌胃与股骨头局部给药对兔激素性股骨头坏死(SONFH)的干预效果。方法取40只成年新西兰白兔,采用注射大肠杆菌内毒素及甲强龙建立SONFH模型。将兔随机分为A、B、C、D四组,每组各10只。A组给予常规饲料喂养,不予特殊处理;B组给予普伐他汀灌胃;C组行股骨头髓芯减压,并在减压通道内植入明胶海绵;D组行髓芯减压后,在股骨头内植入含普伐他汀的明胶海绵。于造模后第10、14周进行髋关节MRI检查,取股骨头进行组织病理学观察,计算各组空骨陷窝率。结果MRI检查显示,造模后第10、14周,A组股骨头大面积骨坏死,关节内有较多积液;B组股骨头坏死面积明显减小,关节内有少量积液;C组股骨头坏死面积较大,关节内有中等量积液;D组股骨头坏死面积显著减小,关节内有少许积液。组织病理学结果显示,A组股骨头可见大量坏死骨组织,脂肪细胞增生明显,有少量新骨形成;B组有少量坏死骨组织及脂肪细胞,可见大量新骨形成;C组有较多坏死骨组织及脂肪细胞增生,可见少量新骨形成;D组骨坏死组织及脂肪细胞明显减少,有大量新骨形成。造模后第10、14周,A组和C组空骨陷窝率均高于B组和D组(P均<0.05),A组与C组比较、B组与D组比较,差异均无统计学意义(P均>0.05)。结论普伐他汀灌胃与股骨头局部给药均能有效干预兔SONFH,促进坏死股骨头的修复。Objective To compare the effects of intragastric administration of pravastatin and topical administration of pravastatin through femoral head on steroid-induced osteonecrosis of femoral head(SONFH)in rabbits.Methods Forty New Zealand rabbits were selected to establish the SONFH model by injecting E.coli endotoxin and methylpredniso⁃lone.After modeling,the rabbits were randomly divided into four groups,with 10 in each group.There was no special treatment for the rabbits in the group A.The rabbits in the group B were treated with water solvent of pravastatin by ga⁃vage.Gelatin sponge was implanted after core decompression of femoral head in the rabbits of the group C.Gelatin sponge with pravastatin was implanted after core decompression of femoral head in rabbits of the group D.Magnetic resonance im⁃aging(MRI)examination of hip joint was performed at 10 and 14 weeks after modeling,and then the femoral heads were obtained for histology observation.The rate of empty lacunae of each femoral head was calculated.Results MRI results showed that there was massive osteonecrosis with joint effusion in the group A at 10 and 14 weeks after establishing the models,respectively.The area of osteonecrosis and joint effusion decreased obviously in the group B.There was a large area of osteonecrosis and much effusion in the group C.The area of osteonecrosis reduced significantly,and at the same time,little joint effusion was found in the group D.Pathological findings showed that in the group A,there was a large amount of necrotic bone tissues in the femoral head,the proliferation of fat cells was obvious,and there was small amount of new bone formation;in the group B,the area of osteonecrosis and number of adipocytes reduced,and there was a large amount of formation of new bone tissues;in the group C,the area of osteonecrosis reduced slightly,the fat cell pro⁃liferation was observed,and only a little new bone formation was found;in the group D,the area of osteonecrosis de⁃creased,the number of adipocytes reduced significa
