摘　　要：目的研究初产妇产褥期抑郁症(PPD)的影响因素及康复护理方法。方法将300例PPD患者随机分为对照组和康复护理组,各150例。对照组给予PPD常规护理,康复护理组在对照组基础上给予康复护理。比较不同临床特征患者的爱丁堡产后抑郁量表(EPDS)评分,分析初产妇PPD发生的影响因素。比较两组患者的护理效果。结果夫妻关系、家庭年收入、产后并发症、孕期焦虑或抑郁为影响初产妇PPD发生的危险因素(P<0.05)。护理后,两组的EPDS评分降低,婴儿照护能力评分和雌二醇水平升高,且康复护理组优于对照组(P<0.05)。结论夫妻关系、家庭年收入、产后并发症、孕期焦虑或抑郁均与初产妇PPD的发生密切相关。康复护理能够有效改善患者的EPDS评分和雌二醇水平,提高婴儿照护能力。Objective To study the influence factors of postpartum depression(PPD)in primipara and rehabilitation nursing methods.Methods A total of 300 patients with PPD were randomly divided into control group and rehabilitation nursing group,with 150 cases in each group.The control group was given PPD routine nursing,and the rehabilitation nursing group was given rehabilitation nursing on the basis of the control group.The Edinburgh postnatal depression scale(EPDS)scores of patients with different clinical characteristics were compared,and the influence factors of PPD in primipara were analyzed.The nursing effects of the two groups were compared.Results Marital relationship,annual family income,postpartum complications,anxiety or depression during pregnancy were risk factors affecting PPD in primipara(P<0.05).After nursing,the EPDS score of the two groups decreased,the infant care ability score and estradiol level increased,and those of the rehabilitation nursing group were better than the control group(P<0.05).Conclusion Marital relationship,annual family income,postpartum complications,anxiety or depression during pregnancy are closely related to the occurrence of PPD in primipara.Rehabilitation nursing can effectively improve the EPDS score and estradiol level of patients,and improve the ability of infant care.
