杨宇教授论湿热咳嗽特征与治疗宜忌

摘　　要：咳嗽的病因病机十分复杂,其中湿热咳嗽在我国南方尤为多见。湿热二邪交织成复杂的矛盾体,清热碍湿,化湿助热,这决定了湿热咳嗽治疗的特殊性。本文总结了杨宇教授对于湿热咳嗽的治疗宜忌,即宜宣展气机、分消湿热、燥运脾湿,忌苦寒冰伏、敛肺止咳、辛燥伤阴。杨宇教授根据四诊尤其是舌象的细节差异,将湿热咳嗽分为湿重热轻、湿热并重、热重湿轻三种证型,分别主以三仁汤、甘露消毒丹、麻杏石甘汤合甘露消毒丹,疗效确切,其经验可供同道借鉴参考。The etiology and pathogenesis of cough is complicated,and it is often the result of a combination of various evils.The dampness-heat cough is especially common in today,because the dampness-heat evil invasion can lead to cough,and other evils are easy to transform into dampness-heat.The damp and heat will be intertwined into complex contradiction.Clearing away heat singly will boost the dampness,and dispelling dampness singly will boost the heat,which determines the particularity of the treatment of dampness-heat cough.The article summarized the treatment compatibility and contraindication of dampness-heat cough of Professor Yang Yu.It is advisable to promote qimovement and separate dampness and heat and dryness the spleen dampness.It should be avoided to abuse the bitter or pungent indiscriminately,or astringe the lung.Those common misunderstandings in the clinic should be corrected.According to the four diagnoses,especially the difference in the details of the tongue manifestation,Professor Yang Yu divides the dampness-heat cough into three types:dampness predominating over heat,equal severity of dampness and heat,heat predominating over dampness,and mainly deal with Three-seeds Decoction,Ganlu Xiaodu Decoction and Maxing Shigan Decoction.These experiences can be used for reference.
