应用速度向量成像技术评价左室构型正常原发性高血压的左房功能

摘　　要：目的应用速度向量成像技术(VVI)评价左室构型正常的原发性高血压(EH)患者左房功能。方法选取左室构型正常EH病组(EH组)30例,正常对照组(NC组)35例。应用VVI左心房分析功能,获取左房容量-时间及其微分变化曲线,记录最大左房容量(LAVmax)、左房主动收缩前容量(LAVpre)、最小左房容积(LAVmin)、峰值充盈率(PFR)、峰值快速排空率(PRER)、峰值主动排空率(PAER)。计算存储容积(RV)、左心房主动排空容积血容量泵(LAAV)、主动射血分数(LVEFact)和管道容积(CV)。分析左房存储、主动泵血和管道功能。结果 EH组患者的LAVmax、LAVpre、PFR、PAER、RV、LAAV明显高于NC组,PRER/PAER明显低于NC组,差异均有统计学意义(t分别=-2.12、-2.15、-3.24、0.87、-2.26、-2.28、2.51,P均<0.05)。结论左室构型正常EH患者存在舒张功能障碍,左房通过增加最大容量、主动泵血容量和储血功能维持左心室充盈。Objective To evaluate left atrial function in essential hypertensive(EH)patients with normal left ventric-ular geometry by velocity vector imaging(VVI).Methods Thirty cases of essential hypertension group(EH group)with normal left ventricular configuration and thirty-five cases of normal control group(NC group)were selected.Use the VVI left atrial analysis function to obtain the left atrial volume-time and its differential curve,and record the maximal left atrial volume(LAVmax),left atrial presystolic volume(LAVpre),minimal left atrial volume(LAVmin),peak filling rate(PFR),peak rapid emptying rate(PRER),peak atrial emptying rate(PAER).Then calculate the reservoir volume(RV),active emptying volume(LAAV),left atrial active emptying fraction(LVEFact)and conduit volume(CV).The storage function of left atrium,the function of active emptying and the function of conduit were analyzed.Results The LAVmax、LAVpre、PFR、PAER、RV、LAAV of the EH group were significantly higher than those of the NC group,and the PRER/PAER was significantly lower than that of the NC group.The differences were statistically significant(t=-2.12,-2.15,-3.24,0.87,-2.26,-2.28,2.51,P<0.05).Conclusion Diastolic dysfunction occurs in EH patients with normal left ve-ntricular configuration.The left atrium maintains left ventricular filling by increasing the maximum volume,active pumping volume,and the function of reservoir volume.
