基于DEA和SFA方法的我国中医类医疗机构卫生资源配置效率评价研究

摘　　要：目的:评价我国中医类医疗机构卫生资源配置效率,促进区域间中医资源配置的协调。方法:通过DEA分析方法,首先对2018年各省市自治区和各地区的中医类医疗机构卫生资源配置的技术效率进行横截面分析,其次对2009-2018年中医类医疗机构资源配置的技术效率进行综合分析;通过SFA分析方法,分析十年间中医类医疗机构卫生资源配置的成本效率并找出影响因子。结果:2018年全国三十个省市自治区中(不含港澳台和西藏),仅有5个实现DEA有效,平均综合技术效率为0.447,平均纯技术效率为0.955,平均规模效率为0.461,超过80%省市自治区规模报酬递减;2018年中医类医疗机构卫生资源配置效率地区差异显著,华南、华东、华北地区(>0.90),西南、西北地区(>0.60),华中地区、东北地区(<0.60);2009年-2018年,十年平均技术效率为0.969,平均成本效率为0.997,平均住院日、病床使用率、卫技人员数是成本效率主要影响因子。结论:2008-2019年,我国中医类医疗机构综合技术效率较低,成本效率较高。中医类医疗机构规模冗余是各省市自治区中医类医疗机构综合技术效率低下主要原因;中医类医疗机构卫生资源配置效率随地区空间分布差异显著;十年间效率随时间序列呈规律性变化。Objective To evaluate the efficiency of health resource allocation of TCM medical institutions,and promote the coordination of health resource allocation in China.Methods Based on DEA,the technical efficiency of health resource allocation of TCM medical institutions in various regions in 2018 was studied with cross-sectional analysis,and the technical efficiency from 2009 to 2018 was comprehensively analyzed.And the cost efficiency of health resource allocation of TCM medical institutions and its influencing factors were studied with SFA method.Results In 2018,only 5 provinces and autonomous regions achieved DEA efficiency,which has average technical efficiency of 0.447,average pure technical efficiency of 0.955,and average scale efficiency of 0.461.More than 80%of them had decreasing returns to scale.The efficiency of health resource allocation of TCM medical institutions had significant differences among different regions.During the 10 years,the average technical efficiency was 0.969,and the average cost efficiency was 0.997.The average duration of hospitalization,bed utilization rate and amount of health personnel were the main influencing factors to cost efficiency.Conclusion The comprehensive technical efficiency of TCM medical institutions was lower,which was mainly resulted from the size of redundancy,while its cost efficiency was high.The efficiency of health resource allocation of TCM medical institutions had significant differences among different regions.The temporal variation of efficiency is regular during ten years.
