摘  要:目的探讨血脂异常患者SLCO1B1基因多态性对辛伐他汀治疗效果的影响及个体化剂量预测模型。方法选择2018年1月~2020年1月我院收治的血脂异常患者192例,采用随机信封法分为模型组和经验组,每组96例。SLCO1B1基因型为AA型、AG型和GG型。模型组按个体给药剂量调整公式给予辛伐他汀,经验组服用辛伐他汀20 mg,1次/晚,2组均服用3个月,比较2组血脂水平变化和不良反应发生率。结果2组治疗后TC、TG、LDL-C水平均较治疗前明显降低,HDL-C水平较治疗前明显升高(P<0.05,P<0.01)。治疗后,经验组GG型患者TC、TG、LDL-C水平明显低于AG型和AA型患者,HDL-C水平明显高于AG型和AA型患者(P<0.05);模型组不同基因型患者TC、TG、LDL-C、HDL-C水平比较,差异无统计学意义(P>0.05)。模型组与经验组不良反应发生率比较,差异无统计学意义(8.3%vs 15.6%,P=0.120)。结论SLCO1B1基因多态性可能影响辛伐他汀的调脂效果,个体化剂量给药可降低SLCO1B1基因多态性对药物敏感性不同的影响。Objective To study the effect of SLCO1B1 gene polymorphism on simvastatin therapy for dyslipidemia patients and the individualized dose prediction model of simvastatin.Methods One hundred and ninety-two elderly dyslipidemia patients admitted to our hospital were divided into model group(n=96)and experimental group by random envelope method.The SLCO1B1 genotypes were divided into AA genotype,AG genotype and GG genotype.The patients in model group were treated with simvastatin according to the established individualized dose formula,and those in experimental group were treated with simvastatin(20 mg/d)for 3 months.The serum level of lipids and the incidence of adverse reactions were compared between the two groups.Results The serum levels of TC,TG,LDL-C and HDL-C were significantly lower and those of HDL-C were significantly higher in two groups after treatment than before treatment(P<0.05,P<0.01).The serum levels of TC,TG,LDL-C were significantly lower while those of HDL-C were significantly higher in GG genotype patients than in AG genotype patients and AA genotype patients after treatment(P<0.05)with no significant difference detected in serum levels of TC,TG,LDL-C and HDL-C in model group and in incidence of adverse reactions between th two groups(P>0.05).Conclusion SLCO1B1 gene polymorphism can affect the lipid-lowering effect of simvastatin.Individualized dose of simvastatin can reduce the effect of SLCO1B1 gene polymorphism on the sensitivity to different drugs in elderly dyslipidemia patients.
