基于信息化的肠内营养耐受动态干预应用于重症患者的效果分析

摘  要:目的探讨重症医学科(intensive care unit,ICU)患者应用基于信息化的肠内营养耐受动态干预的效果。方法选取2018年9月至2020年9月在无锡市第二人民医院ICU住院治疗的重症患者70例,随机数字表法将患者分为对照组和试验组,每组35例。对照组男16例,女19例,年龄(41.28±2.12)岁;试验组男22例,女13例,年龄(43.54±3.12)岁。对照组实施常规肠内营养干预,试验组实施基于信息化的肠内营养耐受动态干预。对比两组患者的喂养情况、不良反应发生率及机体各项指标营养评分。结果试验组患者的喂养不耐受率、喂养中断率均低于对照组[2.86%(1/35)比22.86%(8/35)、2.86%(1/35)比25.71%(9/35)],喂养达标率高于对照组[94.29%(33/35)比51.43%(18/35)],两组比较差异均有统计学意义(均P<0.05)。试验组患者的并发症总发生率低于对照组[5.71%(2/35)比37.14%(13/35)],两组比较差异有统计学意义(χ^(2)=10.267,P=0.001);试验组患者的食欲、活动能力、神经心理及体质量营养状况评分均优于对照组(均P<0.001)。结论在重症患者中应用基于信息化的肠内营养耐受动态干预,显著改善了患者机体营养状态,提高肠内营养的达标率,减少喂养过程中不良反应的发生。Objective To investigate the effect of dynamic intervention of enteral nutrition tolerance based on informatization for ICU severe patients.Methods Seventy severe patients treated at ICU of Wuxi Second People's Hospital from September,2018 to September,2020 were divided into a control group and a trial group by the random number tablemethod,with 30 cases in each group.There were 16males and 19 females in the control group;they were(41.28±2.12)years old.There were 22 males and 13 females in the trial group;they were(43.54±3.12)years old.Routine enteral nutrition intervention was carried out in the control group,and dynamic intervention of enteral nutrition tolerance based on informatization was carried out in the trail group.The feeding status,incidence of adverse reactions,and nutrition scores of body indicators were compared between these two groups.Results The incidences of feeding intolerance and feeding discontinuation in the trial group were lower than those in the control group[2.86%(1/35)vs.22.86%(8/35)and 2.86%(1/35)vs.25.71%(9/35);both P<0.05].The feeding qualified rate in the trial group was higher than that in the control group[94.29%(33/35)vs.51.43%(18/35);P<0.05].The total incidence of complications in the trial group was lower than that in the control group[5.71%(2/35)vs.37.14%(13/35);χ^(2)=10.267,P=0.001].The scores of appetite,activity ability,neuropsychological status,and body weight in the trial group were better than those in the control group(all P<0.001).Conclusion Dynamic intervention of intestinal nutrition tolerance based on informatization for ICU severe patients can significantly improve their nutritional status and qualified rate of enteral nutrition,and reduce the occurrence of adverse reactions during feeding.
