妊娠合并心脏病患者不良妊娠结局的影响因素

摘　　要：目的探讨妊娠合并心脏病患者不良妊娠结局的影响因素。方法回顾性分析2018年1月至2019年12月于南阳市第一人民医院接受治疗的110例妊娠合并心脏病患者的临床资料,观察所有入选者妊娠结局,并收集患者一般资料,分析可能影响妊娠合并心脏病患者不良妊娠结局的相关因素。结果110例患者中妊娠结局良好64例,占58.18%;妊娠结局不良46例,占41.82%。单因素分析显示,妊娠合并心脏病患者发生不良妊娠结局与孕次、分娩孕周无关(P>0.05);妊娠合并心脏病患者发生不良妊娠结局与年龄、是否存在妊娠高血压、心功能分级、心理状况有关(P<0.05)。logistic回归分析显示,年龄≥35岁、有妊娠高血压、心功能分级为Ⅲ~Ⅳ级、心理状况不佳是妊娠合并心脏病患者发生不良妊娠结局的影响因素(OR>1,P<0.05)。结论妊娠合并心脏病患者不良妊娠结局发生率较高,年龄≥35岁、有妊娠高血压、心功能分级为Ⅲ~Ⅳ级、心理状况不佳是妊娠合并心脏病患者发生不良妊娠结局的影响因素。临床应严密监测该症患者妊娠高血压发生状况,尽早采取对应干预措施。同时,尽可能改善患者心功能,并给予心理疏导,减轻患者心理负担,有助于降低不良妊娠结局发生率。Objective To explore the influencing factors of adverse pregnancy outcome in patients with pregnancy combined with heart disease.Methods The clinical data of 110 patients with pregnancy combined with heart disease treated in Nanyang First People’s Hospital from January 2018 to December 2019 were retrospectively analyzed.Their pregnancy outcomes were observed,and the general data of patients were collected.The related factors that might affect the adverse pregnancy outcome of patients with pregnancy combined with heart disease were analyzed.Results Among 110 cases of patients,64 cases had good pregnancy outcome,accounting for 58.18%;46 cases had poor pregnancy outcome,accounting for 41.82%.Univariate analysis showed that adverse pregnancy outcomes were not related to pregnancy times and gestational weeks(P>0.05).The occurrence of adverse pregnancy outcomes in patients with pregnancy combined with heart disease was related to age,with or without gestational hypertension,cardiac function classification and psychological status(P<0.05).Logistic regression analysis showed that age≥35 years old,gestational hypertension,cardiac function gradeⅢ-Ⅳand poor psychological status were the influencing factors of adverse pregnancy outcome in patients with pregnancy combined with heart disease(OR>1,P<0.05).Conclusion The incidence of adverse pregnancy outcomes in patients with pregnancy combined with heart disease is high.Age≥35 years old,gestational hypertension,cardiac function gradeⅢ-Ⅳand poor psychological status are the influencing factors of adverse pregnancy outcomes in patients with pregnancy combined with heart disease.The occurrence of gestational hypertension of patients with this disease should be monitored closely in clinic,and corresponding intervention measures should be taken as soon as possible.At the same time,improving the classification of cardiac function of patients and giving psychological counseling,reducing the psychological burden of patients are helpful to reduce the incidence of adverse
