摘　　要：目的探讨高血压患者血浆肾素活性水平与血钠、尿钠水平的相关性。方法选取2015年6月至2016年10月博爱县人民医院收治的125例高血压患者作为研究对象,按血浆肾素活性水平分为高肾素组(64例,>1.13 ng·mL^(-1)·h^(-1))与低肾素组(61例,<1.13 ng·mL^(-1)·h^(-1))。比较两组24 h收缩压(SBP)、24 h舒张压(DBP)、昼间SBP、夜间SBP、24 h尿钠及血钠水平,分析血浆肾素活性水平与血钠、尿钠水平的相关性。结果高肾素组24 h SBP、夜间SBP、24 h尿钠、血钠水平低于低肾素组(P<0.05);两组24 h DBP、昼间SBP比较,差异无统计学意义(P>0.05);高肾素组空腹血糖水平、总胆固醇(TC)、甘油三酯(TG)水平与低肾素组比较,差异无统计学意义(P>0.05)。两组血压类型比较,差异有统计学意义(P<0.05);血浆肾素活性水平与24 h SBP、夜间SBP、24 h尿钠、血钠水平呈负相关(P<0.05)。经多因素logistic回归方程分析,结果显示,24 h尿钠、血钠、杓型高血压是高血压患者血浆肾素活性水平升高的独立危险因素。结论低血浆肾素活性水平高血压患者血钠、尿钠水平较高,对患者应采取低盐饮食,降低血压,减少心血管事件发生风险。Objective To investigate the correlation between plasma renin activity level and serum sodium and urine sodium levels in patients with hypertension.Methods A total of 125 patients with hypertension in Boai County People’s Hospital from June 2015 to October 2016 were selected as the research objects.According to the plasma renin activity level,they were divided into high renin group(64 cases,>1.13 ng·mL^(-1)·h^(-1))and low renin group(61 cases,<1.13 ng·mL^(-1)·h^(-1)).Compare the 24 h systolic blood pressure(SBP),24 h diastolic blood pressure(DBP),daytime SBP,night SBP,24 h urine sodium and blood sodium levels between the two groups,and the correlations between plasma renin activity levels and blood sodium and urine sodium levels were analyzed.Results The levels of 24 h SBP,night SBP,24 h urine sodium and blood sodium in the high renin group were lower than those of the low renin group(P<0.05).There were no differences between the two groups in 24 h DBP and daytime SBP(P>0.05).The fasting blood glucose levels,total cholesterol(TC)and triglyceride(TG)levels of the renin group had no difference with the low renin group(P>0.05).There was difference between the two groups of blood pressure types(P<0.05).The plasma renin activity level was negatively correlated with 24 h SBP,night SBP,24 h urine sodium,and blood sodium levels(P<0.05).After multivariate logistic regression analysis,the results showed that 24 h urine sodium,blood sodium,and dipper hypertension were independent risk factors for elevated plasma renin activity levels in hypertensive patients.Conclusion Patients with low plasma renin activity levels have high blood sodium and urine sodium levels,and low-salt diet should be used to lower blood pressure and reduce the risk of cardiovascular events.
